摘要

目的 分析床旁超声测量视神经鞘直径(optic nerve sheath diameter,ONSD)诊断中枢神经系统感染患者颅内压(intracranial pressure,ICP)增高的价值.方法 回顾性选取2020年3月-2023年3月徐州医科大学附属医院收治的96例中枢神经系统感染患者的临床资料.根据腰椎穿刺检测的脑脊液压力结果,将入组患者分为ICP正常组(n=45)和ICP增高组(n=51).分析两组患者的ONSD水平差异,绘制受试者操作特征(receiver operating characteristic,ROC)曲线,计算ONSD评估ICP增高的诊断效能.结果 ICP增高组的ONSD为(5.61±0.51)mm,高于ICP正常组的(4.50±0.52)mm,差异有统计学意义(P<0.05).超声检测ONSD对ICP增高的诊断灵敏度、特异度、曲线下面积分别为90.2%、86.7%、0.932,截断值为5.01 mm.结论 ONSD可随着中枢神经系统感染患者ICP增高而增大,床旁即时超声测量ONSD可良好地反映ICP增高水平.

Abstract

Objective To analyze the value of optic nerve sheath diameter(ONSD)measured by bedside ul-trasound in the diagnosis of intracranial pressure(ICP)in patients with central nervous system infection.Meth-ods Retrospectively selected the clinical data of 96 patients with central nervous system infections admitted to the Affiliated Hospital of Xuzhou Medical University between March 2020 and March 2023.They were divided into the group with normal ICP(n=45)and the group with increased ICP(n=51)based on the lumbar puncture detection of cerebrospinal fluid pressure result.The difference in ONSD levels between the two groups was ana-lyzed,and the receiver operating characteristic curve(ROC)was plotted to calculate the diagnostic efficacy of ONSD in evaluating elevated ICP.Results The ONSD of increased ICP group was(5.61±0.51)mm,which was higher than that of normal ICP group(4.50±0.52)mm,and the difference was statistically significant(P<0.05).The diagnostic sensitivity,specificity,and area under the curve of ONSD for increased ICP were 90.2%,86.7%,and 0.932,respectively,with an optimal cutoff value of 5.01 mm.Conclusion ONSD can increase with the in-crease of ICP in patients with central nervous system infection.Real-time ultrasound measurement of ONSD can reflect the increase of ICP well.
