Objective To explore the application value of incoherent motion diffusion-weighted MRI(IVIM-DWI)within voxels in preoperative staging and differentiation of esophageal cancer.Methods A total of 87 patients with suspected esophageal cancer admitted to Dongtai People's Hospital from October 2020 to Au-gust 2023 were selected as study objects.All patients underwent IVIM-DWI scan before operation.With patho-logical examination as the gold standard,the diagnostic efficacy and coincidence rate of IVIM-DWI scan for preoperative staging and differentiation of esophageal cancer were analyzed.Results The results of pathological examination showed that 81 cases were diagnosed as esophageal cancer,and 80 cases were diagnosed by IVIM-DWI scan.The accuracy,sensitivity and specificity of IVIM-DWI were 97.70%,98.77%and 83.33%,respec-tively,and the kappa value was 0.821.By pathological examination,81 cases of esophageal cancer patients,tu-mor node metastasis classification(TNM)stage I 39 cases,TNM stage Ⅱ 30 cases,and TNM stage Ⅲ 12 cases,there were 15 cases of high differentiation,26 cases of moderate differentiation and 40 cases of low differentia-tion.IVIM-DWI scan TNM stage I in 40 cases(97.50%),TNM stage II in 30 cases(100.00%),TNM stage III in 10 cases(91.67%).IVIM-DWI scan showed that 13 cases(86.67%)were highly differentiated,27 cases(96.30%)were moderately differentiated,and 40 cases(100.00%)were poorly differentiated.Conclusion IVIM-DWI has high diagnostic efficacy in preoperative staging and differentiation of esophageal cancer,and can be used as the optimal diagnosis and treatment plan for preoperative staging and differentiation of esopha-geal cancer.
