活血化瘀汤鼻饲+醒脑静注射液静脉滴注+通腑泻热汤灌肠治疗重型颅脑损伤后急性期脑血管痉挛的效果

摘要

目的探讨对重型颅脑损伤后急性期脑血管痉挛患者采用活血化瘀汤鼻饲联合醒脑静注射液静脉滴注+通腑泻热汤灌肠治疗的临床效果.方法回顾性选取2020年10月—2023年10月济南市济阳区中医医院收治的70例重型颅脑损伤后急性期脑血管痉挛患者的临床资料,以治疗方法的不同将患者分为对照组(35例,常规西医治疗)、研究组(35例,在对照组的基础上,采用活血化瘀汤鼻饲+醒脑静注射液静脉滴注+通腑泻热汤灌肠治疗).比较两组Barthel指数、日常生活能力评定量表、格拉斯哥昏迷量表评分、不良反应总发生率.结果治疗后,研究组Barthel指数评分为(82.76±7.85)分、日常生活能力量表评分为(83.29±7.63)分,均高于对照组的(77.39±7.46)分、(78.03±7.69)分,差异有统计学意义(t=2.933、2.872,P均＜0.05).与对照组比较,研究组治疗后格拉斯哥昏迷量表评分更高,不良反应总发生率更低,差异有统计学意义(P均＜0.05).结论对重型颅脑损伤后急性期脑血管痉挛患者,活血化瘀汤+醒脑静注射液+通腑泻热汤的应用可显著改善患者的精神状态并提高生活质量.

Abstract

Objective To explore the clinical effects of nasal feeding treatment with Huoxue Huayu Decoc-tion combined with intravenous drip of Xingnaojing Injection and enema treatment with Tongfu Xiere Decoction in patients with cerebral vasospasm in the acute phase after heavy craniocerebral injuries.Methods The clini-cal data of 70 patients with cerebral vasospasm in the acute phase after heavy craniocerebral injury admitted to Jinan Jiyang District Hospital of Traditional Chinese Medicine from October 2020 to October 2023 were retro-spectively selected.According to the different treatment methods,the patients were divided into control group(35 cases,using conventional Western medicine treatment),the study group(35 cases,on the basis of the control group,using nasal feeding of Huoxue Huayu Decoction to implement,combined with Xingnaojing Injection for intravenous drip+Tongfu Xiere decoction of the enema).The Barthel Index,Activies of Daily Living score,Glasgow Coma Scale score and total incidence of adverse reactions were compared between the two groups.Re-sults After treatment,The Barthel Index score was(82.76±7.85)points,and the Activies of Daily Living score was(83.29±7.63)points in the study group,which were higher than(77.39±7.46)points and(78.03±7.69)points of the control group,and the differences were statistically significant(t=2.933,2.872,both P＜0.05).Compared to the control group,the study group had a higher Glasgow Coma Scale score after treatment and a lower total in-cidence of adverse reactions,and the differences were statistically significant(both P＜0.05).Conclusion The application of Huoxue Huayu Decoction+Xingnaojing Injection+Tongfu Xiere Decoction can significantly im-prove the mental state and quality of life of patients with acute cerebral vasospasm in the acute phase after heavy craniocerebral injuries.
