摘要
目的 探讨关节活动系统联合关节松动术对髌骨骨折术后患者疼痛及活动受限的改善效果.方法 回顾性选取青岛市即墨区人民医院于2020年1月—2023年1月收治的50例髌骨骨折手术治疗患者的临床资料,按照治疗方法的不同分为两组.对照组(n=25)采用关节松动术治疗,观察组(n=25)采用关节活动系统联合关节松动术治疗.比较两组患者术后1、3、6个月的视觉模拟法(Vi-sual Analog Scale,VAS)评分、膝关节功能(Hospital for Special Surgery,HSS)评分、关节活动度(range of motion,ROM)、日常生活活动能力(Activities of Daily Living,ADL)评分.结果 观察组患者术后1、3、6个月的VAS评分分别为(4.24±1.08)分、(2.68±0.87)分、(1.36±0.65)分,低于对照组的(5.32±1.21)分、(3.76±0.98)分、(2.48±0.76)分,差异有统计学意义(t=3.329、4.121、5.600,P均＜0.05).观察组患者术后1、3、6个月的HSS评分、ADL评分及ROM优于对照组,差异有统计学意义(P均＜0.05).结论 关节活动系统联合关节松动术对髌骨骨折术后患者疼痛及活动受限有明显的改善效果,可以提高患者的膝关节功能和日常生活质量.
Abstract
Objective To investigate the improvement effect of joint mobility system combined with joint loosening on the pain and activity limitation of postoperative patellar fracture patients.Methods The clinical data of 50 patients with patellar fracture surgical treatment admitted to Qingdao Jimo District People's Hospital from January 2020 to January 2023 were retrospectively selected,and they were divided into two groups accord-ing to the different treatment methods.The control group(n=25)was treated with joint loosening surgery,and the observation group(n=25)was treated with joint mobility system combined with joint loosening surgery.The Vi-sual Analog Scale(VAS)scores,(Hospital for Special Surgery(HSS)scores,range of motion(ROM),and Activi-ties of Daily Living(ADL)scores of the patients in the two groups were compared at 1,3,and 6 months after Surgery.Results The VAS scores of patients in the observation group were(4.24±1.08)points,(2.68±0.87)points,and(1.36±0.65)points at 1,3,and 6 months after surgery,respectively,which were lower than(5.32±1.21)points,(3.76±0.98)points,(2.48±0.76)points of the control group,and the differences were statistically significant(t=3.329,4.121,5.600,all P＜0.05).The HSS score,ADL score,and ROM of patients in the observa-tion group at 1,3,and 6 months after surgery were better than those of the control group,and the differences were statistically significant(all P＜0.05).Conclusion Joint mobility system combined with joint loosening has a significant improvement effect on the pain level and activity limitation of postoperative patients with patellar fracture,which can improve the patients'knee function and quality of daily life.
