参附养心汤对阳虚水泛型慢性心力衰竭患者心功能及炎症反应的影响

摘要
目的 探讨参附养心汤对阳虚水泛型慢性心力衰竭患者心功能及炎症反应的影响.方法 选取2021年1月—2023年6月南通市通州区中医院收治的86例阳虚水泛型慢性心力衰竭患者为研究对象,按治疗方法不同分为两组,各43例.对照组采取常规治疗,观察组采取常规治疗联合参附养心汤治疗.比较两组治疗效果、心功能、炎症反应与不良反应发生情况.结果 观察组治疗总有效率为95.35%(41/43),高于对照组的81.40%(35/43),差异有统计学意义(χ2=4.074,P<0.05).治疗前,两组心功能指标比较,差异无统计学意义(P均>0.05);治疗后,观察组左室射血分数高于对照组,左室舒张末期内径与左室收缩末期内径低于对照组,差异有统计学意义(P均<0.05).治疗前,两组炎症因子水平比较,差异无统计学意义(P均>0.05);治疗后,观察组各炎症因子水平均低于对照组,差异有统计学意义(P均<0.05).两组不良反应发生率比较,差异无统计学意义(P>0.05).结论 采用参附养心汤治疗阳虚水泛型慢性心力衰竭,可改善患者心功能,减轻炎症反应,效果较佳.
Abstract
Objective To investigate the effect of Shenfu Yangxin Decoction on cardiac function and in-flammatory response in chronic heart failure patients with Yang deficiency and water inundation syndrome.Methods A total of 86 patients with Yang deficiency and water inundation syndrome chronic heart failure admit-ted to Nantong Tongzhou Hospital of Traditional Chinese Medicine from January 2021 to June 2023 were se-lected as the study objects and divided into two groups according to different treatment methods,with 43 cases in each group.The control group was treated with conventional treatment,and the observation group was treated with conventional treatment combined with Shenfu Yangxin Decoction.The therapeutic effect,cardiac function,inflammatory response and adverse reactions of the two groups were compared.Results The total effective rate of the observation group was 95.35%(41/43),which was higher than 81.40%(35/43)of the control group,the difference was statistically significant(χ2=4.074,P<0.05).Before the treatment,two groups of cardiac function indexes were compared,there was no statistically significant difference(all P>0.05).After treatment,the left ventricular ejection fraction of the observation group was higher than that of the control group,and the left ven-tricular end-diastolic diameter and left ventricular end-systolic diameter were lower than those of the control group,and the differences were statistically significant(all P<0.05).Before treatment,there was no statistically significant difference in the levels of inflammatory factors between the two groups(all P>0.05).After treatment,the levels of inflammatory factors in the observation group were lower than those in the control group,and the differences were statistically significant(all P<0.05).There was no statistically significant difference in the inci-dence of adverse reactions between the two groups(P>0.05).Conclusion Shenfu Yangxin Decoction in the treat-ment of chronic heart failure of Yang deficiency and water inundation syndrome can improve the cardiac func-tion of patients,reduce the inflammatory response,and the effect is better.
