老年高龄股骨粗隆间骨折治疗中PFNA内固定术的临床应用价值分析

摘要
目的 分析老年高龄股骨粗隆间骨折(intertrochanteric femoral fracture,IFF)应用股骨近端防旋髓内钉(proximal femoral nail antirotation,PFNA)内固定术治疗的临床价值.方法 选取2020年1月—2022年12月丰县人民医院收治的80例老年高龄IFF患者为研究对象,根据不同的治疗方法分为两组,各40例.对照组采取股骨近段锁定钢板固定治疗,观察组采取PFNA内固定术治疗.比较两组预后指标、髋关节功能及并发症发生情况.结果 观察组术后住院时间、骨折愈合时间均短于对照组,差异有统计学意义(P均<0.05).术后,观察组髋关节功能量表评分均高于对照组,差异有统计学意义(P均<0.05).观察组并发症发生率为5.00%(2/40),低于对照组的20.00%(8/40),差异有统计学意义(χ2=4.114,P<0.05).结论 高龄IFF采用PFNA内固定术治疗,可调节患者髋关节功能,缩短骨折愈合及住院时间,且并发症较少,预后良好.
Abstract
Objective To analyze the clinical value of using proximal femoral anti rotation nail(PFNA)in-ternal fixation for the treatment of intertrochanteric femoral fracture(IFF)in elderly patients.Methods A total of 80 elderly patients with IFF admitted to Fengxian People's Hospital from January 2020 to December 2022 were selected as the research objects.According to different treatment methods,they were divided into two groups,with 40 cases in each group.The control group was treated with proximal femoral locking plate fixation,and the observation group was treated with PFNA internal fixation.The prognostic indicators,hip function and the ouurrence of complications were compared between the two groups.Results The postoperative hospitaliza-tion time and fracture healing time of the observation group were shorter than those of the control group,and the differences were statistically significant(both P<0.05).After operation,the scores of Harris Hip Score in the ob-servation group were higher than those in the control group,and the differences were statistically significant(all P<0.05).The incidence of complications in the observation group was 5.00%(2/40),which was lower than 20.00%(8/40)in the control group,and the difference was statistically significant(χ2=4.114,P<0.05).Conclu-sion PFNA internal fixation for elderly IFF can regulate hip joint function,shorten fracture healing and hospital-ization time,with fewer complications and good prognosis.
