纤维支气管镜对急性重症肺炎并呼吸衰竭治疗的临床疗效分析

摘要
目的 分析对急性重症肺炎并呼吸衰竭患者实施纤维支气管镜治疗的临床疗效.方法 选取2023年1月—2024年1月北京市昌平区医院重症医学科收治的64例急性重症肺炎并呼吸衰竭患者为研究对象,根据治疗方案的不同分为对照组(32例,行常规治疗)和研究组(32例,行纤维支气管镜治疗),对比两组肺通气功能、血气指标.结果 治疗后,与对照组相比,研究组用力肺活量更高,气道峰压、气道阻力更低,差异有统计学意义(P均<0.05).研究组动脉血氧分压为(86.04±4.52)mmHg,血氧饱和度为(99.62±2.14)%,高于对照组的(77.51±4.18)mmHg、(96.47±2.40)%,差异有统计学意义(t=7.837、5.541,P均<0.05).研究组二氧化碳分压低于对照组,差异有统计学意义(P<0.05).结论 在急性重症肺炎并呼吸衰竭患者治疗中,予以纤维支气管镜治疗效果确切.
Abstract
Objective To analyze the clinical effect of fiberoptic bronchoscopy in the treatment of acute se-vere pneumonia with respiratory failure.Methods A total of 64 patients with acute severe pneumonia and respi-ratory failure admitted to the Department of Critical Care Medicine of Changping District Hospital of Beijing from January 2023 to January 2024 were selected as the research objects.According to the different treatment schemes,they were divided into control group(32 cases,routine treatment)and study group(32 cases,fiberoptic bronchoscopy treatment).The pulmonary ventilation function and blood gas indexes were compared between the two groups.Results After treatment,compared with the control group,the study group had higher forced vital ca-pacity,lower airway peak pressure and airway resistance,and the differences were statistically significant(all P<0.05).The arterial partial pressure of oxygen in the study group was(86.04±4.52)mmHg,and the blood oxygen saturation was(99.62±2.14)%,which were higher than those in the control group of(77.51±4.18)mmHg,(96.47±2.40)%,the differences were statistically significant(t=7.837,5.541,both P<0.05).The partial pressure of carbon dioxide in the study group was lower than that in the control group,the difference was statistically signifi-cant(P<0.05).Conclusion In the treatment of acute severe pneumonia with respiratory failure,the effect of fi-beroptic bronchoscopy is definite.
