偏瘫患者肱二头肌长头肌腱炎超声引导下鞘膜液压扩张治疗疗效分析

摘要
目的 探讨偏瘫患者肱二头肌长头肌腱炎超声引导下鞘膜液压扩张治疗的疗效.方法 选取2022年10月—2023年9月滨州市第二人民医院收治的68例偏瘫肱二头肌长头肌腱炎患者为研究对象,根据不同的治疗方式分为扩张组与对照组,各34例.对照组给予塞来昔布口服治疗,扩张组在对照组治疗的基础上给予超声引导下鞘膜液压扩张治疗,比较两组的治疗总有效率、肩关节功能评分、肱二头肌长头肌腱厚度、腱鞘厚度.结果 扩张组的治疗总有效率为97.06%(33/34),高于对照组的82.35%(28/34),差异有统计学意义(χ2=3.981,P<0.05).治疗4周后,两组Constant-Murdey肩关节功能评分均高于治疗前,且扩张组高于对照组,差异有统计学意义(P<0.05).治疗4周后,两组的肱二头肌长头肌腱厚度、腱鞘厚度均减少,且扩张组均少于对照组,差异有统计学意义(P均<0.05).结论 超声引导下鞘膜液压扩张治疗偏瘫患者肱二头肌长头肌腱炎能促进改善肩关节功能,提高总体治疗效果,改善肌腱厚度、腱鞘厚度.
Abstract
Objective To investigate the curative effect of ultrasound-guided sheath hydraulic dilatation in the treatment of hemiplegia patients with long head tendinitis of biceps brachii.Methods From October 2022 to September 2023,68 hemiplegic patients with tendinitis of long head of biceps brachii admitted to Binzhou Sec-ond People's Hospital were selected as the research objects.According to different traetment menthos,they were divided into expansion group and control group,with 34 cases in each group.The control group was treated with celecoxib orally,and the expansion group was treated with ultrasound-guided sheath hydraulic expansion on the basis of the control group.The total effective rate,shoulder function score,biceps longus tendon thick-ness and tendon sheath thickness were compared between the two groups.Results After 4 weeks of treatment,the total effective rate of the expansion group was 97.06%(33/34),which was higher than 82.35%(28/34)of the control group,and the difference was statistically significant(χ2=3.981,P<0.05).After 4 weeks of treatment,the Constant-Murdey shoulder function scores of the two groups were higher than those before treatment,and the expansion group was higher than the control group,the differences were statistically significant(P<0.05).After 4 weeks of treatment,the thickness of the long head of the biceps tendon and the thickness of the tendon sheath in the two groups decreased,and the expansion group was less than the control group,the difference was statisti-cally significant(both P<0.05).Conclusion Ultrasound-guided sheath hydraulic dilatation in the treatment of hemiplegic patients with biceps tendinitis can promote the improvement of shoulder joint function,improve the overall treatment effect,and improve the thickness of the tendon and the thickness of the tendon sheath.
