腹腔镜胆囊切除术治疗急性结石性胆囊炎的效果分析

摘要
目的 分析急性结石性胆囊炎患者采取腹腔镜胆囊切除术的临床效果.方法 选取2021年2月—2023年2月聊城市中医医院收治的60例急性结石性胆囊炎患者为研究对象,根据不同的治疗方法分为两组,每组30例,研究组采取腹腔镜胆囊切除术,对照组采取开腹胆囊切除术,比较两组临床指标、炎性因子水平、胃肠激素指标、并发症发生率治疗效果.结果 研究组术中出血量、腹腔引流量均少于对照组,手术时间、胃肠功能恢复时间、住院时间均短于对照组,差异有统计学意义(P均<0.05).研究组肿瘤坏死因子-α、C-反应蛋白、白细胞介素-6水平均低于对照组,差异有统计学意义(P均<0.05).研究组胃泌素高于对照组,差异有统计学意义(P<0.05).研究组并发症发生率为3.33%(1/30),低于对照组的23.33%(7/30),差异有统计学意义(χ2=5.192,P<0.05).结论 急性结石性胆囊炎患者实施腹腔镜胆囊切除术效果显著,可缩短患者手术时间,减轻患者机体炎性反应,降低并发症发生率.
Abstract
Objective To analyze the clinical effect of laparoscopic cholecystectomy in patients with acute calculous cholecystitis.Methods A total of 60 patients with acute calculous cholecystitis admitted to Liaocheng Hospital of Traditional Chinese Medicine from February 2021 to February 2023 were selected as the research objects.According to different treatment methods,they were divided into two groups,30 cases in each group.The study group was treated with laparoscopic cholecystectomy,and the control group was treated with open cholecystectomy.The clinical indexes,inflammatory factor levels,gastrointestinal hormone indexes and compli-cation rates of the two groups were compared.Results The intraoperative blood loss and abdominal drainage vol-ume in the study group were less than those in the control group,and the operation time,gastrointestinal func-tion recovery time and hospitalization time were shorter than those in the control group,the differences were sta-tistically significant(all P<0.05).The levels of tumor necrosis factor-α,C-reactive protein and interleukin-6 in the study group were lower than those in the control group,and the differences were statistically significant(all P<0.05).The gastrin in the study group was higher than that in the control group,and the difference was statistically significant(P<0.05).The incidence of complications in the study group was 3.33%(1/30),which was lower than 23.33%(7/30)in the control group,and the difference was statistically significant(χ2=5.192,P<0.05).Conclu-sion Laparoscopic cholecystectomy is effective in patients with acute calculous cholecystitis,which can shorten the operation time,reduce the inflammatory response of patients and reduce the incidence of complications.
