输尿管软镜在肾结石治疗中的作用分析

摘要
目的 分析在肾结石治疗中使用输尿管软镜的临床价值.方法 回顾性选取2020年12月—2023年 12月仙桃第三医院收治的 80例肾结石患者的临床资料,根据手术方法不同分为两组,每组40例,研究组采用输尿管软镜碎石术,对照组采用经皮肾镜碎石术.比较两组临床指标、一次性结石清除率、肾功能指标、炎症指标和不良反应.结果 研究组手术时间、术中出血量和术后下床时间均优于对照组,差异有统计学意义(P均<0.05).研究组一次性结石清除率为95.00%(38/40),高于对照组的80.00%(32/40),差异有统计学意义(χ2=4.114,P<0.05).术后,两组血清肌酐、尿微量白蛋白水平均升高,相比较于对照组,研究组水平更低,差异有统计学意义(P均<0.05).术后,两组白细胞介素-6、降钙素原、及C反应蛋白水平均升高,但相比较于对照组,研究组水平更低,差异有统计学意义(P均<0.05).研究组不良反应总发生率低于对照组,差异有统计学意义(P<0.05).结论 输尿管软镜治疗肾结石的效果显著,其手术时间较短,患者术后下床时间缩短,有利于术后恢复,且肾功能指标改善较佳,术后炎症指标上升幅度较小,术后创伤较轻,且并发症较少.
Abstract
Objective To analyze the clinical value of flexible ureterosape in the treatment of kidney stones.Methods Clinical data of 80 patients with kidney stones admitted to Xiantao Third Hospital from December 2020 to December 2023 were retrospectively selected,and they were divided into two groups with 40 cases in each group according to different surgical methods.Flexible ureterosape lithotripsy was performed in the study group,and percutaneous nephrolithotripsy was performed in the control group.Clinical indexes,one-time stone clearance rate,renal function indexes,inflammatory indexes and adverse reactions were compared between the two groups.Results The operation time,intraoperative blood loss and postoperative getting out of bed time of the study group were better than those of the control group,and the differences were statistically significant(all P<0.05).The one-time stone clearance rate in the study group was 95.00%(38/40),higher than 80.00%(32/40)in the control group,the difference was statistically significant(χ2=4.114,P<0.05).After operation,serum creati-nine and urinary microalbumin levels were increased in both groups,but the levels in the study group were lower than those in the control group,the differences were statistically significant(all P<0.05).After operation,the levels of interleukin-6,procalcitonin,and C-reactive protein in both groups were increased,but the levels in the study group were lower than those in the control group,and the differences were statistically significant(all P<0.05).The total incidence of adverse reactions in the study group was lower than that in the control group,the difference was statistically significant(P<0.05).Conclusion Flexible ureteroscope has a significant effect on the treatment of kidney stones,with short operation time and shortened postoperative time for patients to get out of bed,which is conducive to postoperative recovery,and better improvement of renal function indicators,small increase of postoperative inflammation indicators,less postoperative trauma and less complications.
