冠心病检查诊断中常规心电图与运动平板心电图检查的作用

摘要
目的 分析冠状动脉粥样硬化性心脏病(简称冠心病)检查诊断中常规心电图与运动平板心电图(exercise tablet electrocardiogram,TET)检查的作用.方法 选取2020年6月—2023年6月滕州市精神卫生中心检查的60例疑似冠心病患者为研究对象,所有入选者均进行常规心电图与TET检查.以冠脉造影(coronary angiography,CAG)结果作为金标准,比较常规心电图与TET检查在冠心病中的诊断价值.结果 60例疑似冠心病患者经CAG检查发现,37例确诊为冠心病,23例为非冠心病.TET检查诊断冠心病的准确度为95.00%(57/60)、特异度为95.65%(22/23)、灵敏度为94.59%(35/37)、阴性预测值为91.67%(22/24)、阳性预测值为97.22%(35/36),均高于常规心电图的70.00%(42/60)、65.22%(15/23)、72.97%(27/37)、60.00%(15/25)、77.14%(27/35),差异有统计学意义(χ2=12.987、4.973、6.366、6.640、4.777,P均<0.05).结论 冠心病检查诊断中TET诊断价值优于常规心电图,可提升诊断效能,便于临床作出准确判断.
Abstract
Objective To analyze the effect of routine electrocardiogram and exercise tablet electrocardio-gram(TET)in the diagnosis of coronary heart disease.Methods A total of sixty patients with suspected coronary heart disease who were examined in Tengzhou Mental Health Center from June 2020 to June 2023 were selected as the study objects.All the subjects were examined by routine electrocardiogram and TET.The results of coro-nary angiography(CAG)were used as the gold standard to compare the diagnostic value of routine electrocardio-gram and TET in coronary heart disease.Results 60 patients with suspected coronary heart disease were found by CAG examination,37 cases were diagnosed with coronary heart disease,23 cases were non-coronary heart disease.The accuracy,specificity,sensitivity,negative predictive value and positive predictive value of TET in the diagnosis of coronary heart disease were 95.00%(57/60),95.65%(22/23),94.59%(35/37),91.67%(22/24)and 97.22%(35/36),respectively,which were higher than 70.00%(42/60),65.22%(15/23),72.97%(27/37),60.00%(15/25)and 77.14%(27/35)of routine electrocardiogram,and the differences were statistically signifi-cant(χ2=12.987,4.973,6.366,6.640,4.777,all P<0.05).Conclusion The diagnostic value of TET in the diag-nosis of coronary heart disease is better than that of conventional electrocardiogram,which can improve the diag-nostic efficiency and facilitate accurate clinical judgment.
