摘要

目的 分析急性脑梗死一站式CT血管成像(CT angiography,CTA)和CT灌注成像(CT per-fusion imaging,CTP)结合诊断的临床价值.方法 选择2022年5月—2023年5月聊城市人民医院收治的100例疑似急性脑梗死患者为研究对象,均接受一站式CTA+CTP检查.以全脑数字减影血管造影检查为金标准,分析一站式CTA+CTP检查对急性脑梗死的检出情况、诊断效能、诊断符合率,比较急性脑梗死确诊患者梗死区、缺血半暗带区的患侧、健侧相关参数.结果 全脑数字减影血管造影检查检出75例为急性脑梗死,一站式CTA+CTP检查共检出72例急性脑梗死.一站式CTA+CTP检查诊断急性脑梗死的灵敏度为90.67%(68/75),特异度为84.00%(21/25)、准确度为89.00%(89/100),kappa值为0.718.一站式CTA+CTP检查对轻度狭窄、中度狭窄、重度狭窄、完全闭塞的诊断符合率分别为94.12%(32/34)、96.30%(26/27)、100.00%(9/9)、100.00%(5/5).梗死区患侧脑血流量、脑血容量均低于对健侧,差异有统计学意义(P均<0.05);缺血半暗带区患侧脑血流量、脑血容量均低于健侧,且达峰时间高于健侧,差异有统计学意义(P均<0.05).结论 一站式CTA+CTP检查在诊断急性脑梗死中具有较高的诊断效能及诊断符合率,可以对梗死区、缺血半暗带区准确快速区分,并有效评估动脉狭窄程度与闭塞情况.

Abstract

Objective To analyze the clinical value of one-stop CT angiography(CTA)combined with CT perfusion imaging(CTP)in the diagnosis of acute cerebral infarction.Methods A total of 100 patients with sus-pected acute cerebral infarction admitted to Liaocheng People's Hospital from May 2022 to May 2023 were se-lected as the research objects.All patients underwent one-stop CTA+CTP examination,and the whole brain digital subtraction angiography was used as the gold standard.The detection,diagnostic efficacy and diagnostic coincidence rates of one-stop CTA+CTP examination for acute cerebral infarction were analyzed,and the re-lated parameters of the affected side and the healthy side of the infarct area and ischemic penumbra area in pa-tients with acute cerebral infarction were compared.Results Among the 75 cases of acute cerebral infarction were detected by whole brain digital subtraction angiography,and 72 cases of acute cerebral infarction were de-tected by one-stop CTA+CTP.The sensitivity of one-stop CTA+CTP in the diagnosis of acute cerebral infarc-tion was 90.67%(68/75),the specificity was 84.00%(21/25),the accuracy was 89.00%(89/100),and the kappa value was 0.718.The diagnostic coincidence rates of one-stop CTA+CTP examination for mild stenosis,moder-ate stenosis,severe stenosis and complete occlusion were 94.12%(32/34),96.30%(26/27),100.00%(9/9)and 100.00%(5/5),respectively.The cerebral blood flow and cerebral blood volume of the affected side in the infarc-tion area were lower than those of the healthy side,and the differences were statistically significant(both P<0.05).The cerebral blood flow and cerebral blood volume of the affected side in the ischemic penumbra were lower than those of the healthy side,and the peak time was higher than that of the healthy side,the differences were statistically significant(all P<0.05).Conclusion One-stop CTA+CTP examination has high diagnostic ef-ficiency and diagnostic coincidence rate in the diagnosis of acute cerebral infarction.It can accurately and quickly distinguish the infarct area and ischemic penumbra area,and effectively evaluate the degree of arterial stenosis and occlusion.
