摘要

目的 探讨经食道超声心动图(transesophageal echocardiography,TEE)对非瓣膜性心房颤动患者的临床诊断价值.方法 选取2022年4月—2023年8月黔南州中医医院收治的100例非瓣膜性心房颤动患者作为观察组,另选取同期50名体检健康者作为对照组,两组均接受TEE检查,对比两组左心耳容积和血流动力学指标,同时比较观察组中不同左心耳功能患者的左心耳容积和血流动力学指标.结果 观察组左心耳最小容积(left atrial appendage minimum volume index,LAAVmin)、左心耳最大容积(left atrial appendage maximum volume index,LAAVmax)分别为(5.51±1.03)mL、(8.68±2.27)mL,均高于对照组[(1.04±0.32)mL、(5.42±1.46)mL],差异有统计学意义(t=29.929、9.236,P均<0.05).观察组左心耳射血分数(left atrial appendage ejection fraction,LAAEF)、左心耳最大排空速度(left atrial appendage peak flow emptying velocity,LAA-PEV)、左心耳容积变化率(left atrial appendage vulum change rate,LAA-VCR)、左心耳排空指数(left atrial appendage volume change rate,LAAEI)均低于对照组,差异有统计学意义(P均<0.05).不同左心耳功能患者的LAAVmin、LAAVmax、LAAVCR、LAA-PEV、LAA-EF、LAAEI比较,差异有统计学意义(P均<0.05).结论 在非瓣膜性心房颤动患者的诊断过程中,TEE可进一步评估其左心耳容积和左心耳功能,且通过TEE观察患者的左心耳容积及血流动力学情况可进一步评估左心耳功能.

Abstract

Objective To investigate the clinical diagnostic value of transesophageal echocardiography(TEE)in patients with nonvalvular atrial fibrillation.Methods From April 2022 to August 2023,100 patients with nonvalvular atrial fibrillation admitted to Qiannan Hospital of Traditional Chinese Medicine were selected as the observation group,and 50 healthy subjects were selected as the control group.Both groups received TEE examination.The left atrial appendage volume and hemodynamic indexes of the two groups were compared,and the left atrial appendage volume and hemodynamic indexes of patients with different left auricular function in the observation group were compared.Results The left atrial appendage minimum volume index(LAAVmin)and left atrial appendage maximum volume index(LAAVmax)in the observation group were(5.51±1.03)mL and(8.68±2.27)mL,which were higher than those in the control group[(1.04±0.32)mL and(5.42±1.46)mL],and the differences were statistically significant(t=29.929,9.236,both P<0.05).The left atrial appendage ejection fraction(LAAEF),left atrial appendage peak flow emptying velocity(LAA-PEV),left atrial appendage volume change rate(LAA-VCR)and left atrial appendage evacuation index(LAAEI)in the observation group were lower than those in the control group,and the differences were statistically significant(all P<0.05).The levels of LAAVmin,LAAVmax,LAAVCR,LAA-PEV,LAA-EF and LAAEI among patients with different left atrial appen-dicular function were compared,and the differences were statistically significant(all P<0.05).Conclusion In the diagnosis of patients with nonvalvular atrial fibrillation,TEE can further evaluate the left atrial appendage volume and left atrial appendage function,and the left atrial appendage function can be further evaluated by ob-serving the left atrial appendage volume and hemodynamics of patients through TEE.
