阴道超声在子宫内膜疾病中的诊断价值分析

摘要
目的 研究阴道超声运用于子宫内膜疾病中的诊断效能.方法 选取2017年1月—2023年1月西安市北方医院收治的80例疑似子宫内膜疾病患者为研究对象,所有患者均接受腹部、阴道超声诊断,以宫腔镜病理诊断作为金标准,分析不同诊断方法的诊断效能及诊断符合率.结果 80例疑似子宫内膜疾病患者的病理诊断结果显示,阳性74例,阴性6例.腹部超声检出阳性64例,阴性16例.阴道超声检出阳性72例,阴性8例.阴道超声的灵敏度[94.59%(70/74)]、特异度[66.67%(4/6)]、准确度[92.50%(74/80)]均高于腹部超声[81.08%(60/74)、33.33%(2/6)、77.50%(62/80)],差异有统计学意义(χ2=17.515、5.166、22.279,P均<0.05).阴道超声对子宫内膜息肉及黏膜下子宫肌瘤的诊断符合率高于腹部超声,差异有统计学意义(P均<0.05).结论 临床在开展子宫内膜疾病诊断时可选择阴道超声,诊断价值较高.
Abstract
Objective To study the diagnostic efficacy of vaginal ultrasound in endometrial diseases.Meth-ods A total of 80 patients with suspected endometrial diseases admitted to Xi'an North Hospital from January 2017 to January 2023 were selected as the research objects.All patients underwent abdominal and vaginal ultra-sound diagnosis.Hysteroscopic pathological diagnosis was used as the gold standard to analyze the diagnostic ef-ficacy and diagnostic coincidence rate of different diagnostic methods.Results The pathological diagnosis re-sults of 80 patients with suspected endometrial diseases showed that 74 cases were positive and 6 cases were negative.Abdominal ultrasonography was positive in 64 cases and negative in 16 cases.Vaginal ultrasonogra-phy was positive in 72 cases and negative in 8 cases.The sensitivity[94.59%(70/74)],specificity[66.67%(4/6)]and accuracy[92.50%(74/80)]of vaginal ultrasound were higher than those of abdominal ultrasound[81.08%(60/74),33.33%(2/6),77.50%(62/80)],and the differences were statistically significant(χ2=17.515,5.166,22.279,all P<0.05).The diagnostic coincidence rates of vaginal ultrasonography for endometrial polyps and sub-mucosal myoma were higher than those of abdominal ultrasonography,and the differences were statistically sig-nificant(both P<0.05).Conclusion In clinical diagnosis of endometrial diseases,vaginal ultrasound can be se-lected,which has high diagnostic value.
