超声联合多层螺旋CT用于宫内节育器异位的临床诊断价值

摘要
目的 探究超声联合多层螺旋CT(multi-slice spiral CT,MSCT)扫描用于宫内节育器异位的临床诊断价值.方法 选取2020年10月—2023年10月无锡市第八人民医院收治的95例疑似宫内节育器异位患者为研究对象,均行超声及MSCT检查.以宫腔镜手术结果为金标准,分析超声、MSCT及联合检查对宫内节育器异位的诊断效能;比较两种检查方法及联合检查对不同类型、不同部位宫内节育器异位的诊断符合率.结果 联合检查结果与宫腔镜手术结果的一致性(kappa=0.670)高于超声(kappa=0.310)、MSCT(kappa=0.411).超声、MSCT及联合检查诊断宫内节育器异位的灵敏度、特异度比较,差异无统计学意义(P均>0.05);诊断准确度比较,差异有统计学意义(P<0.05).联合检查诊断的灵敏度、准确度高于超声检查,差异有统计学意义(P均<0.05).联合检查对宫内节育器下移的诊断符合率高于超声,差异有统计学意义(P<0.05).联合检查对子宫肌壁异位的诊断符合率高于超声,差异有统计学意义(P<0.05).结论 超声联合MSCT对宫内节育器异位具有较高的诊断价值,且对不同类型及不同部位宫内节育器异位的检出率高于超声、MSCT单独检查.
Abstract
Objective To explore clinical diagnostic value of ultrasound combined with multi-slice spiral CT(MSCT)for ectopia of intrauterine device.Methods Ninety-five cases of suspected ectopic intrauterine de-vice patients admitted to Wuxi Eighth People's Hospital from October 2020 to October 2023 were selected as the study objects,all of whom underwent ultrasound and MSCT examination.Using hysteroscopic surgery re-sults as the gold standard,the diagnostic efficacy of ultrasound,MSCT and combined examination in the diagno-sis of ectopic intrauterine device was analyzed.The diagnostic coincidence rate of different types and different of ectopic parts was compared between the two methods and combined examination.Results The consistency of the combined examination results with that of hysteroscopy surgery(kappa=0.670)was higher than that of ultra-sound(kappa=0.310)and MSCT(kappa=0.411).There were no significant differences in sensitivity and speci-ficity of ultrasonography,MSCT and combined examination in diagnosing ectopic intrauterine device(both P>0.05).The difference of diagnostic accuracy was statistically significant(P<0.05).The diagnostic sensitivity and accuracy of combined examination were higher than those of ultrasound examination,and the differences were statistically significant(both P<0.05).The diagnostic coincidence rate of combined examination was higher than that of ultrasound,and the difference was statistically significant(P<0.05).The diagnostic coincidence rate of combined examination was higher than that of ultrasound,the difference was statistically significant(P<0.05).Conclusion Ultrasound combined with MSCT has a high diagnostic value for ectopic intrauterine device,and the detection rate of different types and different parts of ectopicintrauterine device is higher than that of ultra-sound and MSCT alone.
