多层螺旋CT冠脉造影在心脏血管狭窄中的应用价值

摘要
目的 探讨多层螺旋CT冠脉造影诊断心脏血管狭窄疾病的效果与应用价值.方法 选取2018年9月—2023年12月建湖县人民医院接诊的疑似心脏血管狭窄患者134例,均接受多层螺旋CT冠脉造影检查,以数字减影血管造影结果作为金标准,计算多层螺旋CT冠脉造影诊断的灵敏度、特异度及准确度,同时分析多层螺旋CT冠脉造影诊断狭窄位置、狭窄程度的诊断符合率.结果 134例疑似心脏血管狭窄患者经数字减影血管造影检查,90例(67.16%)患者确诊心脏血管狭窄情况,44例(32.84%)患者无心脏血管狭窄问题,多层螺旋CT冠脉造影诊断的灵敏度为97.78%(88/90)、特异度为100.00%(44/44),准确度为98.51%(132/134),kappa值为0.966.进一步分析显示,多层螺旋CT冠脉造影技术诊断心脏血管狭窄位置及狭窄程度的诊断符合率较高.结论 心脏血管狭窄可通过多层螺旋CT冠脉造影技术诊断,具有较高的灵敏度、特异度及准确度,为后续疾病的诊断与治疗提供可靠影像学支持.
Abstract
Objective To explore the effect and application value of multi-slice spiral CT coronary angiog-raphy in diagnosis cardiacvascular stenosis.Methods A total of 134 patients with suspected cardiacvascular stenosis were selected from Jianhu County People's Hospital from September 2018 to December 2023.All of them were examined by Multi-slice spiral CT coronary angiography,and the results of digital subtraction angi-ography were used as the gold standard.The diagnostic sensitivity,specificity and accuracy of multi-slice spiral CT coronary angiography were calculated,and the diagnostic coincidence rate of multi-slice spiral CT coronary angiography in diagnosing stenosis location and degree was analyzed.Results Among 134 suspected patients with cardiac vascular stenosis,90 patients(67.16%)were diagnosed with cardiac vascular stenosis,and 44 pa-tients(32.84%)were not diagnosed with cardiac vascular stenosis,accounting for 32.84%.The sensitivity,specificity and accuracy of multi-slice spiral CT coronary angiography were 97.78%(98/90),100.00%(44/44)and 98.51%(132/134),the kappa value was 0.96 Further analysis showed that multi-slice spiral CT coronary angiography had a high diagnostic coincidence rate for the location and degree of cardiac vessel stenosis.Con-clusion Cardiac vascular stenosis can be diagnosed by multi-slice spiral CT coronary angiography,which has high sensitivity,specificity,and accuracy,providing reliable imaging support for the diagnosis and treatment of subsequent diseases.
