正压呼吸机在基层医院阻塞性睡眠呼吸暂停低通气综合征患者治疗中的意义

摘要
目的:评估具有远程监测和调控功能的正压呼吸机在阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者中的临床应用价值.方法:选取2020 年 1 月至2021 年 12 月辽宁省鞍山市中心医院耳鼻喉科诊断为OSAHS并开具呼吸机治疗处方的 80 例患者作为研究对象,按照随机数字表法分为观察组和对照组,每组40 例.观察组采用具有远程监测和调控功能的无创正压通气(NPPV)呼吸机治疗,对照组采用存储卡模式呼吸机治疗,定期随访复诊结合自行管理模式,进行为期1 年固定时间点随访,比较戴机依从性.戴机治疗 12 个月后,比较呼吸机平均使用时间及治疗前后Epworth嗜睡量表(ESS)评分.结果:12 个月后观察组坚持戴机例数为34 例,对照组坚持戴机例数为 24 例.在坚持戴机 12 个月的患者中观察组平均每次开机使用时间为(6.38±1.60)h,对照组为(6.15±1.45)h,差异无统计学意义(P>0.05).2 组患者中戴机治疗 12 个月后的ESS评分观察组为(4.25±2.26)分,对照组为(6.37±3.28)分,观察组ESS评分明显低于对照组,差异有统计学意义(P<0.05).结论:将具有远程监测和调控功能的呼吸机引入到基层医院的OSAHS患者呼吸机治疗管理中,在治疗的前期更有利于提高患者的戴机率,提高患者呼吸机使用的依从性,改善OSAHS的白天嗜睡症状.
Abstract
Objective:To evaluate the clinical application value of a positive pressure ventilator with remote monitoring and regu-lation function in patients with obstructive sleep apnea syndrome(OSAHS).Methods:A total of 80 patients diagnosed with OS-AHS and prescribed ventilator treatment at the Otolaryngology Department of Anshan Central Hospital in Liaoning Province from January 2020 to December 2021 were selected as the study subjects.They were randomly divided into an observation group and a control group,with 40 patients in each group.The observation group was treated with non-invasive positive pressure ventilation(NPPV)ventilator with remote monitoring and regulation functions,while the control group was treated with memory card mode ventilator.Regular follow-up and self management mode were used for 1-year fixed time point follow-up to compare machine com-pliance.After12 months of mechanical therapy,compare the average duration of ventilator use and the Epworth Sleepiness Scale(ESS)score before and after treatment.Results:After 12 months,the observation group had 34 cases of persistent use of ma-chines,while the control group had 24 cases.Among the patients who persisted in using the device for12 months,the average time for each startup and use in the observation group was(6.38±1.60)hours,while in the control group,it was(6.15±1.45)hours.There was no statistically significant difference in data between the two groups(P>0.05).After 12 months of machine therapy,the ESS scores of the observation group and the control group were(4.25±2.26)and(6.37±3.28)respectively.The ESS scores of the observation group were significantly lower than those of the control group,with a statistically significant differ-ence(P<0.05).Conclusion:Introducing a ventilator with remote monitoring and control functions into the management of OS-AHS patients'ventilator treatment in grassroots hospitals is more conducive to improving the patient's chances of carrying the ventilator in the early stages of treatment,improving the patient's compliance with ventilator use,and improving the daytime drows-iness symptoms of OSAHS.
