超声引导下臂丛神经阻滞联合右美托咪定麻醉对上肢损伤患者血流动力学、睡眠情况及不良反应的影响

摘要
目的:探究超声引导下臂丛神经阻滞联合右美托咪定麻醉对上肢损伤患者血流动力学、睡眠情况及不良反应的影响.方法:选取2023 年 1 月至2023 年8 月福建福州市第二医院收治的上肢损伤患者72 例作为研究对象,按照随机数字表法分为对照组和观察组,每组36 例.对照组患者接受超声引导下臂丛神经阻滞麻醉,观察组患者接受超声引导性下臂丛神经阻滞联合右美托咪定麻醉,比较2 组患者以下指标:血流动力学指标、疼痛情况、睡眠情况、不良反应发生情况.结果:比较2 组患者血流动力学指标,T2-T4 阶段观察组患者心率(HR)、平均动脉压(MAP)明显低于对照组(P<0.05).观察组疼痛情况评分明显较对照组低,差异有统计学意义(P<0.05).观察组睡眠质量评分明显较对照组低,差异有统计学意义(P<0.05).观察组不良反应发生情况较高,但2 组患者差异无统计学意义(P>0.05).结论:对上肢损伤患者实行超声引导下臂丛神经阻滞联合右美托咪定麻醉,对其血流动力学产生影响较小,能有效缓解其疼痛感,可减少对其睡眠产生的影响,较为安全,故具有较高临床推广应用价值.
Abstract
Objective:To investigate the effects of ultrasound-guided brachial plexus block combined with dexmedetomidine anes-thesia on hemodynamics,sleep status and adverse reactions in patients with upper limb injury.Methods:A total of 72 patients with upper limb injury admitted to our hospital during 2023.1 to 2023.8 were divided into two groups by completely randomized grouping,namely control group(36 cases)and observation group(36 cases).The control group received ultrasound-guided brachi-al plexus block anesthesia,and the observation group received ultrasound-guided brachial plexus block combined with dexmedeto-midine anesthesia.The following indexes were compared between the two groups:hemodynamic indexes,pain,sleep status,and occurrence of adverse reactions.Results:Compared the hemodynamic indexes of the two groups,the heart rate(HR)and mean ar-terial pressure(MAP)in the observation group at stage T2-T4 were significantly lower than those in the control group(P<0.05).The pain score of the two groups was significantly lower in the observation group(P<0.05).The sleep quality score of the two groups was significantly lower in the observation group(P<0.05).The occurrence of adverse reactions between the two groups was higher in the observation group,but there was no significant difference between the two groups(P>0.05).Conclusion:Ultra-sound-guided subbrachial plexus block combined with dexmedetomidine anesthesia for patients with upper limb injury has little effect on the hemodynamics of patients,can effectively relieve the pain of patients,can reduce the impact on patients'sleep,and is relatively safe,so it has high clinical application value.
