摘要
目的:分析叙事护理干预对血液透析患者心理健康与睡眠质量的影响.方法:选取2022 年 1 月至2023 年 5 月陆军第七十三集团军医院肾内科行血液透析治疗的患者66 例作为主要对象,采用随机数字表法分为对照组和观察组,每组33 例.对照组给予常规护理干预,观察组在对照组基础上给予叙事护理干预.采用匹兹堡睡眠质量指数(PSQI)比较2 组患者干预前后睡眠质量的变化,采用汉密尔顿焦虑量表(HAMA)、汉密尔顿抑郁量表(HAMD)比较 2 组患者干预前后焦虑、抑郁状态的改善效果,采用生命质量测定量表(QLQ-C30)比较 2 组患者干预前后生命质量的变化.结果:干预后,观察组HAMA评分、HAMD评分、PSQI评分显著低于对照组,观察组QLQ-C30 评分显著高于对照组,差异均有统计学意义(均P＜0.05).结论:将叙事护理运用在血液透析患者的护理工作中能够获得显著性优势,不仅可以改善患者的不良情绪,又可以提高其睡眠质量和生命质量,有借鉴意义和临床推广价值.
Abstract
Objective:To analyze the impact of narrative nursing intervention on the psychological health and sleep quality of he-modialysis patients.Methods:A total of 66 patients who underwent hemodialysis treatment in the Department of Nephrology at the 73rd Army Hospital of the Army from January 2022 to May 2023 were selected as the main subjects.They were randomly divided into an observation group(n =33)and a control group(n =33)using a random number table method.The control group received routine nursing intervention,while the observation group received narrative nursing intervention on the basis of the control group.The Pittsburgh Sleep Quality Index(PSQI)was used to compare the changes in sleep quality between two groups of patients before and after intervention.The Hamilton Anxiety Scale(HAMA)and Hamilton Depression Rating Scale(HAMD)were used to compare the improvement effects of anxiety and depression status between the two groups of patients before and after interven-tion.The Quality of Life Scale(QLQ-C30)was also used Compare the changes in quality of life between two groups of patients be-fore and after intervention.Results:After intervention,the HAMA score,HAMD score,and PSQI score of the observation group were significantly lower than those of the control group,while the QLQ-C30 score of the observation group was significantly higher than that of the control group.The differences between the two groups were statistically significant(P＜0.05).Conclusion:The application of narrative nursing in the nursing work of hemodialysis patients can achieve significant advantages.It can not only im-prove the patient's negative emotions,but also improve their sleep quality and quality of life,which has reference significance and clinical promotion value.
