综合护理干预对胆囊结石腹腔镜手术患者术后疼痛、睡眠质量的影响

摘要
目的:分析综合护理对行胆囊结石腹腔镜手术患者术后早期预后的作用.方法:选取2022 年 7 月至2023 年 6 月福建中医药大学附属人民医院普通外科收治的接受腹腔镜手术治疗的胆囊结石患者 100 例作为研究对象,按照随机抽签法分为对照组和观察组,每组50 例.对照组患者在围手术期实施常规护理,观察组患者在围手术期开展综合护理方案.比较2 组患者术后6h、1 d、2d视觉模拟量表(VAS)分值、肛门排气时间、肠鸣音恢复时间、食欲恢复时间和术后1 d、3d、5d的匹兹堡睡眠质量指数.结果:观察组患者的术后 1 d、2 d VAS分值比对照组下降,差异有统计学意义(P＜0.05);观察组患者术后 1 d、3d、5d匹兹堡睡眠质量指数较对照组下降,差异有统计学意义(P＜0.05);观察组患者术后肛门排气时间、肠鸣音恢复时间、食欲恢复时间较对照组下降,差异有统计学意义(P＜0.05).结论:综合护理方案可促进行胆囊结石腹腔镜手术患者术后早期胃肠功能的恢复,减轻其术后的疼痛症状,预防睡眠质量障碍的发生.
Abstract
Objective:To analyze the effect of comprehensive nursing on the early postoperative prognosis of patients undergoing laparoscopic cholecystectomy.Methods:From July 2022 to June 2023,100 patients with cholecystolithiasis who received laparo-scopic surgery in our hospital were randomly divided into two groups,including routine group and experimental group,with 50 ca-ses in each group.Routine nursing was carried out in the routine group during perioperative period,and comprehensive nursing scheme was carried out in the experimental group during perioperative period.The visual analogue scale scores,anal exhaust time,bowel sounds recovery time,appetite recovery time at 6 hours,1 day and 2 days after operation,and Pittsburgh sleep quality index at 1 days,3 day and 5 days after operation were compared between the two groups.Results:The VAS scores of patients in the ex-perimental group were significantly lower than those in the routine group at 1 day and 2 days after operation(P＜0.05).The Pitts-burgh sleep quality index of patients in the experimental group were significantly lower than those in the routine group at the 1,3 and 5 days after operation(P＜0.05).The postoperative anal exhaust time,bowel sounds recovery time and appetite recovery time in the experimental group were significantly lower than those in the routine group(P＜0.05).Conclusion:Comprehensive nursing scheme can promote the early recovery of gastrointestinal function of patients undergoing laparoscopic cholecystectomy,relieve the pain symptoms of patients and prevent the occurrence of sleep quality disorders.
