探讨营养联合护理干预对终末期肾病维持性血液透析患者睡眠和生命质量的影响

摘要
目的:探讨营养联合护理干预对终末期肾病维持性血液透析患者睡眠和生命质量的影响.方法:选取 2022 年 3月至2023 年3 月福建省厦门大学附属中山医院肾内科进行血液透析的患者 86 例作为研究对象,按照抽签法随机分为对照组和观察组,每组43 例.对照组在透析时实施常规护理,观察组实施营养联合护理干预.比较2 组营养指标、睡眠质量、肾功能指标、生命质量.结果:护理前2 组患者血红蛋白、前白蛋白、白蛋白比较,差异无统计学意义(P＞0.05);护理后观察组营养指标水平高于对照组,差异有统计学意义(P＜0.05).护理前 2 组阿森斯睡眠质量量表(AIS)、失眠频率比较,差异无统计学意义(P＞0.05);护理后观察组AIS、失眠频率低于对照组,差异有统计学意义(P＜0.05).护理前2 组尿素氮、血肌酐比较,差异无统计学意义(P＞0.05);护理后观察组尿素氮、血肌酐、血磷低于对照组,差异有统计学意义(P＜0.05).护理前2 组生命质量综合评定问卷(GQOL-74)比较,差异无统计学意义(P＞0.05);护理后观察组高于对照组,差异有统计学意义(P＜0.05).结论:营养联合护理干预可保障终末期肾病患者的血液透析效果,改善患者营养指标,使睡眠质量与生命质量受到疾病的影响降低.
Abstract
Objective:To explore the effect of nutritional combined nursing intervention on sleep and quality of life in mainte-nance hemodialysis patients with end-stage renal disease.Methods:A total of 86 patients who underwent hemodialysis in our hos-pital from March 2022 to March 2023 were selected as the study subjects.Randomly divide 43 cases into groups by drawing lots.The control group received routine care during dialysis,while the observation group received nutritional combined care inter-vention.Compare the nutritional indicators,sleep quality,renal function indicators,and quality of life between the two groups.Results:The difference in hemoglobin,prealbumin,and albumin between the two groups of patients before nursing care was not significant(P＞0.05);the level of nutritional indicators in the observation group after nursing was higher than that in the control group,the difference was statistically significant(P＜0.05).There was no significant difference(P＞0.05)between the two groups before nursing in terms of AIS and insomnia frequency;After nursing,the frequency of AIS and insomnia in the obser-vation group was lower than that in the control group(P＜0.05).There was no significant difference in urea nitrogen and blood creatinine between the two groups before nursing(P＞0.05);After nursing,the urea nitrogen,bloodcreatinine and and serium in-organic phosphorus levels in the observation group were lower than those in the control group,the difference was statistically sig-nificant(P＜0.05).There was no significant difference(P＞0.05)in the comparison of the comprehensive quality of life assess-ment questionnaire(GQOL-74)between the two groups before nursing care;After nursing,the observation group was higher than the control group,the difference was statistically significant(P＜0.05).Conclusion:Nutritional combined nursing intervention can ensure the effectiveness of hemodialysis in end-stage renal disease patients,improve their nutritional indicators,and reduce the impact of diseases on sleep quality and quality of life.
