疼痛干预对腹腔镜下直肠癌切除术后患者疼痛及睡眠质量的影响

目的:分析疼痛干预在腹腔镜下直肠癌切除术后患者的应用效果.方法:选取2021年9月至2023年6月泉州市第一医院收治的直肠癌患者56例作为研究对象,随机分为观察组和对照组,每组28例.对照组术后采用患者自控静脉镇痛(PCIA),观察组在PCIA的基础上接受综合疼痛干预.比较2组术后疼痛程度、非切口疼痛发生率、PCIA按压次数、睡眠质量.结果:观察组术后6 h、12 h、24 h及48 h疼痛视觉模拟评分(VAS)低于对照组(P＜0.05),术后24 h非切口疼痛发生率较低(P＜0.05),术后24 h内PCIA按压次数较少.观察组术后1 d睡眠效率指数高于对照组,觉醒指数较低(P＜0.05).观察组术后匹兹堡睡眠质量指数显著低于对照组(P＜0.05).结论:综合疼痛干预可减少直肠癌患者腹腔镜术后疼痛,减少睡眠障碍.
Abstract
Objective:To analyze the effect of pain management in patients after laparoscopic rectal cancer resection.Methods:A total of 56 patients with rectal cancer were enrolled and randomly divided into a study group(n=28)and a control group(n=28).The control group received patient-controlled intravenous analgesia(PCIA)after surgery,while the study group received comprehensive pain management based on PCIA.The degree of postoperative pain,incidence of non-incision pain,number of PCIA compressions,and sleep quality were compared.Results:The VAS of the study group was significantly lower at 6 h,12 h,24 h,and 48 h after surgery(P＜0.05),and the incidence of non-incision pain was lower at 24 h after surgery(P＜0.05).And times of PCIA pressing within 24 hours significantly decreased.The sleep efficiency index of the study group was higher than that of con-trol group 1 d after surgery,and the awakening index was lower(P＜0.05).The study group's postoperative Pittsburgh Sleep Quality Index score was significantly lower than that of the control group(P＜0.05).Conclusion:Comprehensive pain manage-ment can reduce postoperative pain and sleep disorders in patients undergoing laparoscopic rectal cancer resection.
