摘要
目的:探究乌拉地尔注射液联合酒石酸美托洛尔片治疗原发性高血压合并心力衰竭的疗效及对睡眠质量的影响.方法:选取2021年4月至2022年4月福建省霞浦县医院心血管内科收治的原发性高血压合并心力衰竭患者120例为研究对象.根据用药不同分为对照组和观察组,每组60例.对照组应用酒石酸美托洛尔片治疗,观察组应用乌拉地尔注射液联合酒石酸美托洛尔片治疗.观察总有效率、治疗前后血压、睡眠质量[匹兹堡睡眠质量指数(PSQI)]评分、心功能指标[左心室射血分数(LVEF)、左心室舒张末期内径(LVEDD)、左心室舒张末期容积(LVEDV)]、血红素加氧酶-1(HO-1)、一氧化碳(CO)及不良反应发生率变化,评估疗效.结果:观察组总有效率98.33％,高于对照组86.67％(P＜0.05).治疗后观察组收缩压、舒张压均低于对照组(P＜0.05);治疗后观察组PSQI评分低于对照组(P＜0.05).治疗后观察组LVEF高于对照组(P＜0.05);LVEDD、LVEDV均低于对照组(P＜0.05);治疗后,观察组血清HO-1水平低于对照组(P＜0.05);CO水平高于对照组(P＜0.05).2组不良反应发生率差异无统计学意义(P＞0.05).结论:乌拉地尔注射液联合酒石酸美托洛尔片治疗原发性高血压合并心力衰竭,可较好地控制患者血压,改善心功能及睡眠质量,且不会明显增加不良反应.
Abstract
Objective:To study the efficacy of urapidil injection combined with metoprolol tartrate tablets in the treatment of pri-mary hypertension complicated with heart failure,and to evaluate its impact on sleep quality.Methods:A total of 120 patients with primary hypertension complicated with heart failure,treated in Xiapu County Hospital,from April 2021 to April 2022,were select-ed for this study.According to the difference of medication,they were divided into control group and observation group,with 60 ca-ses in each group.The control group was treated with metoprolol tartrate tablets,while the observation group was treated with ura-pidil injection combined with metoprolol tartrate tablets.The total efficacy rate,blood pressure before and after treatment,sleep quality[as assessed by the Pittsburgh Sleep Quality Index(PSQI)],cardiac function indexes[left ventricular ejection fraction(LVEF),left ventricular end-diastolic diameter(LVEDD),left ventricular end-diastolic volume(LVEDV)],levels of heme oxy-genase-1(HO-1),carbon monoxide(CO)and the incidence of adverse reactions were used to evaluate the curative effect.Results:The observation group had a higher total efficacy rate(98.33％)compared to the control group(86.67％)(P＜0.05).Post-treatment systolic blood pressures and diastolic blood pressures in the observation group were lower than those in the control group(P＜0.05).The PSQI score of the observation group was lower than that of the control group after treatment(P＜0.05).Furthermore,LVEF was higher in the observation group(P＜0.05),while LVEDD and LVEDV were significantly lower compared to the control group(P＜0.05).Serum HO-1 levels in the observation group were lower(P＜0.05),and the levels of serum CO in the observation group were higher compared to the control group(P＜0.05).Comparing the incidence of adverse re-actions between the two groups,the observation group was lower than that of the control group,but there was no significant differ-ence between the two groups(P＞0.05).Conclusion:The combination of urapidil injection and metoprolol tartrate tablets effec-tively controls blood pressure,improves cardiac function,and enhances sleep quality in patients with primary hypertension compli-cated with heart failure,without significantly increasing the adverse reactions compared to monotherapy.
