Correlation Analysis of Monitoring Parameters Between Central Apnea and Polysomnography in Children with Obstructive Sleep Apnea

目的:研究睡眠呼吸暂停(OSA)患儿中枢性呼吸暂停与多导睡眠监测各指标的相关性.方法:选取2020年12月至2022年9月华北理工大学附属医院呼吸与睡眠监测中心进行整夜呼吸睡眠监测的患儿,收集与整理监测数据,分析中枢性呼吸暂停指数(CAI)与多导睡眠监测其他参数相关性.结果:867例患儿,男558例(64.4%),女309例(35.6%);平均年龄(7.6±2.8)岁.27.8%(241/867)的患儿 1<CAI<5,0.9%(8/867)的患儿 CAI>5.患儿 CAI 随着阻塞性睡眠呼吸暂停低通气指数(OAHI)的增加而增加,与年龄、体质量指数、胸围、最低血氧呈负相关关系.年龄是CAI的主要影响因素且对其有负向预测作用(β=0.006 3,P<0.05),混合型呼吸暂停指数(MAI)是CAI的主要影响因素,对其有正向预测作用(β=0.509,P<0.001).结论:OSA儿童中CAI与年龄呈显著负相关,且是中枢性呼吸暂停的影响因素,与OSA严重程度呈显著正相关,间接通过MAI影响中枢性呼吸暂停,肥胖及血氧饱和度不是其影响因素.
Abstract
Objective:To study the correlation between central apnea and polysomnography monitoring in children with obstruc-tive sleep apnea(OSA).Methods:Children who underwent overnight respiratory and sleep monitoring in the Respiratory and Sleep Monitoring Center of the Affiliated Hospital of North China University of Science and Technology from December 2020 to September 2022 were selected,and the monitoring data were collected and sorted,and the correlation between central apnea index(CAI)and other parameters of polysomnography monitoring was analyzed.Results:A total of 867 children were enrolled,558 ca-ses(64.4%)male,309 cases(35.6%)female;the average age was(7.6±2.8)years.27.8%(241/867)of the children had 1<CAI<5,and 0.9%(8/867)of the children had CAI>5.CAI increased with the obstructive apnea-hypopnea index(OAHI),and was negatively correlated with age,body weight,chest circumference and minimum blood oxygen levels.Age was the main in-fluencing factor of CAI,predicting a negative effect(β=0.006 3,P<0.05).Mixed apnea index(MAI)was another significant influencing factor of CAI and predicted it positively(β=0.509,P<0.001).Conclusion:CAI is negatively correlated with age in children with OSA,and is an influencing factor in central apnea,which is positively correlated with the severity of OSA.CAI indi-rectly affects central apnea through MAI,but obesity and blood oxygen saturation are not influencing factors.
