认知行为干预联合健康教育对急性脑梗死偏瘫患者睡眠质量、负性情绪与遵医行为的影响

摘要
目的:分析认知行为干预(CBT)联合健康教育在急性脑梗死偏瘫患者中的应用效果.方法:选取2023年4月至2023年7月四川大学华西厦门医院收治的急性脑梗死偏瘫患者80例作为研究对象,按照随机数字表法分为对照组和观察组,每组40例.对照组接受常规护理,观察组同步接受CBT与基于跨理论模型的健康教育.比较2组干预后焦虑与抑郁情绪、睡眠障碍、康复训练依从性.结果:观察组干预后汉密尔顿抑郁量表评分(HAMD)、汉密尔顿焦虑量表(HA-MA)与阿森斯失眠量表(AIS)评分均低于对照组,差异有统计学意义(P＜0.05);观察组干预后睡眠障碍发生率低于对照组,康复训练完全依从率较高,差异有统计学意义(P＜0.05).结论:CBT联合健康教育可改善急性脑梗死偏瘫患者睡眠质量、情绪障碍,提升康复训练依从性.
Abstract
Objective:To analyze the impact of cognitive behavioral therapy(CBT)combined with health education in acute cere-bral infarction patients complicated with hemiplegia.Methods:A total of 80 patients with hemiplegia caused by acute cerebral in-farction admitted to West China Xiamen Hospital of Sichuan University from April 2023 to July 2023 were randomly divided into study group(n=40)and control group(n=40).The control group received routine nursing,while the study group received CBT and trans-theoretical model based health education simultaneously.Anxiety and depression,sleep disorder,and exercise adherence after intervention were compared.Anxiety was evaluated by HAMA,depression was evaluated by HAMD,and sleep quality was e-valuated by AIS.Results:After intervention,HAMD,HAMA and AIS scores of study group were lower than those of control group(P＜0.05),the incidence of sleep disorder was lower,and the rate of complete compliance to exercise was significantly higher(P＜0.05).Conclusion:CBT combined with health education can improve sleep quality and emotional disorder in acute cerebral infarction patients complicated with hemiplegia,and enhance exercise adherence.
