摘要
许铣教授从事皮肤科临床、科研工作60余年,中西医结合治疗玫瑰痤疮经验丰富.许铣教授认为,肝郁血热、脾胃湿热为玫瑰痤疮的核心病机.治疗应以中药内服为主,结合疾病分型分期辨证论治,红斑与毛细血管扩张期的基本病因病机为肝失疏泄,郁而化热,基本治法为凉血疏肝兼以养阴清热;丘疹脓疱期主要以脾胃运化失司、脾胃湿热为主,治法为清热燥湿、解毒散结.病程日久者往往合并血瘀和湿热.外治中药常选择具有清热凉血、燥湿、解毒之药效及抑菌、控脂之药理作用的中药.同时,可以根据疾病的临床表现选用西药辅助治疗,临床疗效显著,附验案1例加以佐证.
Abstract
Professor Xu Xian has been engaged in clinical and scientific research in dermatology for over 60 years,with rich experience in the treatment of rosacea through the combination of traditional Chinese and Western medi-cine.Professor Xu believes that liver depression and blood heat,as well as spleen and stomach damp-heat are the core pathogenesis of rosacea,the treatment of which should be based on oral administration of traditional Chinese medicine,com-bined with syndrome differentiation for disease types and stages.The basic etiology and pathogenesis of erythema and telan-giectasia are liver dysfunction,stagnation,and heat accumulation,and the basic treatment is to cool blood and soothe the liver,as well as to nourish Yin and clear heat.The papule and pustule stage is mainly characterized by the loss of spleen and stomach circulation,dominated by dampness and heat in the spleen and stomach.The treatment method is to clear heat,dry dampness,detoxify,and disperse nodules.Patients with a prolonged course of disease often suffer from blood stasis and humid fever.Traditional Chinese medicine for external treatment often chooses those that have the effects of clearing heat,cooling blood,drying dampness,detoxifying,as well as the pharmacological effects of antibacterial activity and lipid control.At the same time,western medicine can be used as an adjuvant treatment based on the clinical manifestations of the disease,with significant clinical efficacy.One case is attached as evidence.
