心可舒片改善射血分数保留心力衰竭大鼠心脏功能及其作用机制探讨

目的 观察心可舒片对射血分数保留心力衰竭(Heart failure with preserved ejection fraction,HFpEF)大鼠心脏结构和功能的影响.方法 将30只自发性高血压大鼠(Spontaneous hypertension rat,SHR)分为SHR组、HFpEF组、心可舒组(334.8 mg/kg),10只Wistar-Kyoto(WKY)大鼠为对照组.心可舒组给予相应药物灌胃,其余组给予等量生理盐水,连续4周.测量血压和心脏重量指数(Heart mass index,HMI);超声心动图检测大鼠左心房内径(Left atrial diameter,LAD)、室间隔厚度(Interventricular septal thickness,IVST)、左室舒张末期内径(Left ven-tricular end diastolic diameter,LVEDd)、左室舒张末期后壁厚度(Left ventricular posterior wall thickness,LVPWd)、E/A、E/E'等容舒张时间(Isovolumic relaxation time,IVRT)、左室射血分数(Left ventricular ejection fraction,LVEF);酶联免疫吸附测定法检测血清B型脑尿钠肽(Brain natriuretic peptide,BNP)、血管紧张素Ⅱ(Angiotensin Ⅱ,Ang 11)、醛固酮(Aldosterone,ALD)、可溶性生长刺激表达基因 2 蛋白(Soluble growth stimulation expressed gene 2 pro-tein,sST2)含量;HE及Masson染色观察心肌组织病理变化.结果 与WKY组比较,HFpEF模型组血压升高,HMI增加,差异有统计学意义(P<0.05);LAD、LVEDd、IVST、LVPWd、E/A、E/E'和IVRT增加,差异有统计学意义(P<0.05),LVEF无明显变化,差异无统计学意义(P>0.05);病理观察显示左心房与左心室心肌细胞增大和心肌纤维化;血清BNP、Ang Ⅱ、ALD、sST2含量升高,差异有统计学意义(P<0.05),提示造模成功.与HFpEF模型组比较,心可舒组收缩压降低,HMI减小,差异有统计学意义(P<0.05);LAD、LVEDd、IVST、LVPWd、E/A、E/E'和IVRT减小,差异有统计学意义(P<0.05);病理观察显示心可舒组左心房和心室心肌细胞减小及纤维化改善;血清BNP、Ang Ⅱ、ALD、sST2含量降低,差异有统计学意义(P<0.05).结论 心可舒片能改善HFpEF模型大鼠心房及左心室的结构和功能,其机制可能与调控肾素-血管紧张素-醛固酮系统及sST2表达,从而改善心肌肥厚和纤维化相关.
Effects of Xinkeshu Tablet on Cardiac Function in Rats with Heart Failure with Pre-served Ejection Fraction and Its Mechanism
Objective To observe the effects of the Xinkeshu(XKS)Tablet on cardiac structure and function in rats with heart failure with preserved ejection fraction(HFpEF).Methods Thirty spontaneously hypertensive rats(SHRs)were divided into the SHR group,HFpEF group,and XKS group(334.8 mg/kg),and ten Wistar Kyoto(WKY)rats were used as the control group.The XKS group was given the corresponding drugs by gavage,and the other groups were given the same amount of normal saline for four weeks.Blood pressure and heart mass index(HMI)were measured.Echocardiography was used to detect the left atrial diameter(LAD),interventricular septal thickness(IVST),left ventricular end-diastolic diameter(LVEDd),left ventricular posterior wall thickness(LVWPd),E/A,E/E',isovolumetric relaxation time(IVRT),and left ventricular ejection fraction(LVEF).The serum levels of brain natriuretic peptide(BNP),angiotensin ar ejection fraction(LVEF thickness(IVST),left ventricular end-diastolic diameter(LVEDd),left ventricular posterior wa immu-nosorbent assay.HE and Masson staining were used to observe the pathological changes in myocardial tissue.Results Compared with the WKY group,the blood pressure and HMI of the HFpEF model group were increased,and the difference was statistically significant(P<0.05).LAD,LVEDd,IVST,LVPWd,E/A,E/E',and IVRT were increased(P<0.05),but LVEF did not change significantly,with the difference not statistically significant(P>0.05).Pathological observation showed that the left atrium and left ventricle had enlarged cardiomyocytes and myocardial fibrosis.The serum levels of BNP,Ang Ⅱ,ALD,and sST2 were increased(P<0.05),suggesting that the model was successfully established.Compared with the HFpEF group,systolic blood pressure and HMI were decreased in the XKS group(P<0.05).LAD,LVEDd,IVST,LVPWd,E/A,E/E',and IVRT were decreased(P<0.05).The pathological observation showed a reduction in car-diomyocytes and an improvement in myocardial fibrosis of the left atrium and ventricle.The serum levels of BNP,Ang Ⅱ,ALD,and sST2 were decreased(P<0.05).Conclusion XKS Tablet can improve the structure and function of the left a-trium and ventricle in HFpEF rats,and its mechanism may be related to regulating the renin-angiotensin-aldosterone system and the expression of sST2,thereby improving myocardial hypertrophy and fibrosis.
