Efficacy of Modified Huangqi Guizhi Wuwu Decoction on Treatment of Diabetic Ar-teriosclerosis Obliterans of Lower Extremity and Its Effect on Vascular Endothelial Func-tion
Objective To explore the efficacy of modified Huangqi Guizhi Wuwu Decoction in the treatment of di-abetic arteriosclerosis obliterans of the lower extremity and its effect on vascular endothelial function.Methods A total of 102 cases of patients with diabetic arteriosclerosis obliterans of the lower extremity featuring Qi deficiency and blood stasis who visited the First Affiliated Hospital of Heilongjiang University of Chinese Medicine from February 2020 to February 2023 were selected as the research subjects.They were divided into a control group and an observation group using a ran-dom number table method,with 51 cases in each group.The control group was treated with conventional Western medicine,while the observation group was treated with Huangqi Guizhi Wuwu Decoction on the basis of the control group.After two-month treatment,the clinical efficacy,adverse reaction,traditional Chinese medicine(TCM)syndrome scores,arterial ima-ging indicators of the lower extremity,hemorheological indicators[whole blood high shear viscosity,whole blood low shear viscosity,plasma viscosity,and hematocrit],and blood lipid indicators[total cholesterol(TC),triglycerides(TG),low-density lipoprotein cholesterol(LDL-C),and high-density lipoprotein cholesterol(HDL-C)]were observed and com-pared before and after treatment between two groups.Results After treatment,the lower limb numbness,decreased skin temperature,intermittent claudication,resting pain score,and total score of the two groups of patients were significantly re-duced,and the difference was statistically significant(P<0.05).The TCM syndrome score and total score in the observa-tion group were significantly lower than those in the control group(P<0.05).After treatment,the inner diameters of the femoral artery,popliteal artery,and posterior tibial artery in the two groups of patients increased significantly.The thickness of the intima of the left and right femoral arteries decreased significantly,and the size of the plaque decreased significantly(P<0.05).The inner diameter values of the femoral artery,popliteal artery,and posterior tibial artery in the observation group were significantly higher than those in the control group,and the plaques were significantly smaller than those in the control group(P<0.05).However,there was no statistically significant difference in the thickness of the intima of the left and right femoral arteries between the two groups of patients(P>0.05).After treatment,TC,TG,LDL-C,whole blood high shear viscosity,whole blood low shear viscosity,and plasma viscosity of the two groups were significantly reduced,while HDL-C and hematocrit levels were significantly increased(P<0.05).In addition,the levels of TC,TG,LDL-C,whole blood high shear viscosity,whole blood low shear viscosity,and plasma viscosity in the observation group were lower than those in the control group,while the levels of HDL-C and hematocrit were higher than those in the control group(P<0.05).After treatment,the total effective rate of the observation group was 90.20%(46/51),which was significantly higher than the control group(62.75%,32/51)(P<0.05).During the treatment process,neither group of patients showed significant adverse reactions,and there were no significant abnormalities in blood and urine routine,electrocardiogram,and liver and kidney function tests before and after treatment.Conclusion Modified Huangqi Guizhi Wuwu Decoction can ef-fectively improve peripheral hemodynamics and vascular endothelial function in the treatment of diabetic arteriosclerosis ob-literans of the lower extremity featuring Qi deficiency and blood stasis and alleviate clinical symptoms and signs.It has a definite therapeutic effect,and medication is safe and reliable.
