Clinical Efficacy of Biling Weitong Granules Combined with Quadruple Therapy in Treating Helicobacter Pylori-Positive Chronic Atrophic Gastritis Patients and Its Protective Effect on Gastric Mucosa
Objective To investigate the efficacy of Biling Weitong granules combined with quadruple therapy in treating patients with Helicobacter Pylori(HP)-positive chronic atrophic gastritis(CAG)and its protective effect on the gastric mucosa. Methods A total of 112 HP-positive CAG patients who were hospitalized or treated as outpatients at The Northern Jiangsu People's Hospital from June 2022 to June 2023 were included in the study.Patients were randomly divid-ed into two groups using a computerized random number table method:a control group and a study group, with 56 patients in each. The control group received standard Western medicine quadruple therapy, while the study group received Biling Weitong granules in addition to the quadruple therapy. Both groups were treated for 8 weeks.Clinical efficacy,HP eradica-tion rate, and adverse reactions were observed and compared between the two groups.Evaluated traditional Chinese medi-cine(TCM)syndrome scores, assessed the pathological conditions of gastric mucosal, detected gastrointestinal hormones[gastrin(GAS),pepsinogen Ⅰ(PGⅠ),pepsinogen Ⅱ(PGⅡ)],as well as measured inflammatory factors[interleukin-1 β(IL-1 β),interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)]before and after treatment. Results After treat-ment,the study group had a significantly higher clinical cure rate of 25.00%(14/56)and a total effective rate of 94.64%(53/56)compared to the control group's 14.29%(8/56)and 80.36%(45/56),respectively,with statistically significant differences(P<0.05).After treatment, the TCM syndrome cure rate in the study group was 30.36%(17/56),and the to-tal effective rate was 98.21%(55/56),both significantly higher than those in the control group, which were 19.64%(11/56)and 82.14%(46/56),respectively,with statistically significant differences(P<0.05).The HP eradication rate in the study group was 94.64%(53/56),significantly higher than the control group's 76.79%(43/56),with a statistically sig-nificant difference(x2=11.594,P=0.001).After treatment,TCM syndrome scores and gastric mucosal pathology scores significantly decreased in both groups compared to before treatment, with statistically significant differences(P<0.05);the TCM syndrome scores and gastric mucosal pathology scores of the study group were significantly lower compared with the control group(P<0.05).After treatment, the GAS and PG Ⅱ of the two groups were lower than before treatment, while the PG Ⅰ and PG Ⅰ/PG Ⅱ ratio were higher than before treatment, with statistically significant differences(P<0.05);the study group also showed lower levels of GAS and PG Ⅱ,as well as higher levels of PG Ⅰ and the PG Ⅰ/PG Ⅱ ratio com-pared to the control group, with statistically significant differences(P<0.05).Additionally,serum levels of IL-1β,IL-6,and TNF-α were significantly reduced in both groups after treatment, with statistically significant differences(P<0.05);the study group showed significantly lower levels of these inflammatory markers than the control group, with statisti-cally significant differences(P<0.05).There was no statistically significant difference in adverse reactions between the two groups(x2=0.117,P=0.733).Conclusion Biling Weitong granules combined with quadruple therapy can improve clinical efficacy and HP eradication rate in HP-positive CAG patients, while also protecting the gastric mucosa. Its mecha-nism may be related to the regulation of gastrointestinal hormone levels and the inhibition of inflammatory cytokines.
