摘要
文章总结了丁丽仙教授从心肾论治绝经生殖泌尿综合征(Genitourinary syndrome of menopause,GSM)的临床经验.丁丽仙教授认为GSM的核心病机是阴阳失和、心肾不交,主要表现为生殖道、泌尿道萎缩以及性功能障碍等症状和体征.丁丽仙教授遵循中医"辨证论治""病证结合"的原则,运用丁氏妇科经验方参麦黄连阿胶汤、三甲复脉汤加减、一贯煎等化裁内服,以泻南补北、交通心肾,育阴潜阳、养血柔肝,滋肾养阴、疏肝解郁为治疗大法;同时重视心理疏导、生活调摄,以使"肝肾精气旺,肝肾之气自舒,心之火不旺,肾之水不亏".
Abstract
This paper summarizes the clinical experience of Professor Ding Lixian in treating genitourinary syn-drome of menopause(GSM)based on the heart and kidney theory.Professor Dingbelieves that the core pathogenesis of the disease is the disharmony between yin and yang and disharmony between the heart and kidney.Meanwhile,the disease fea-tures symptoms and signs of reproductive tract and urinary tract atrophy,and sexual dysfunction.According to the guiding principles of"treatment based on syndrome differentiation"and"integration of disease and evidence",Ding's gynecologi-cal experience formula Shenmai Coptis and Donkey-hide Gelatin Combination is employed,and Sanjia Fumai Decoction is adopted for addition or reduction.Yiguan decoction and other medicines are taken orally to reduce the south and reinforce the north,thus restoring normal coordination between heart and kidney.Additionally,yin is nourished and excessive yang is suppressed to nourish blood and livers.Meanwhile,the kidney and yin are nourished to soothe the liver and relieve depres-sion,which serves as a sound treatment method.At the same time,this paper pays attention to psychological counseling and lifestyle adjustment to nourish the essential qi of livers and kidneys and soothe the qi of livers and kidneys.Additionally,the heart fire is not excessive,without the lack of kidney edema.
