摘要
目的：探讨瑞芬太尼复合异丙酚全身麻醉在老年患者手术中的临床效果。方法从我院2013年6月至2014年6月实施全身麻醉的老年手术患者中随机性抽取100例进行研究和分析，并将研究对象随机的分为研究组和对照组，每组各50例。研究组50例患者给予瑞芬太尼复合异丙酚全身麻醉，对照组50例患者给予芬太尼复合异丙酚全身麻醉。比较2组麻醉效果。结果2组患者手术时间在80～195min，2组患者在手术时间上比价无明显差异（P>0.05）；研究组发生不良反应率10%，明显低于对照组的不良反应率36.00%（P<0.05）；研究组的苏醒时间明显低于对照组，组间数据差异比较具有统计学意义（P<0.05）。结论对于老年手术患者，在手术中采用瑞芬太尼或芬太尼复合异丙酚用全身麻醉维持均能满足手术要求，瑞芬太尼具有术后苏醒快的优点，但是术后镇痛效果不佳。
Abstract
Objective: To investigate the remifentanil combined with propofol for total intravenous anesthesia clinical effect in elderly patients during surgery.Methods: from 2013 June to 2014 June in our hospital implementation of randomness extraction of elderly surgical patients with general anesthesia in 100 cases of research and analysis, and the study subjects were randomly divided into study group and control group, 50 cases in each group.Two groups of patients with remifentanil and propofol in anesthesia induction stage, on the basis of this, the study group of 50 patients received remifentanil combined with propofol for total intravenous anesthesia in the maintenance phase, a control group of 50 patients in the maintenance phase given fentanyl and propofol total intravenous anesthesia.Comparison of two groups of patients before surgery, surgery and postoperative changes in heart rate, blood pressure changes, and record the quality index of patients with adverse reactions rate, pain reaction and wake after testing and recording.Results: the study group of adverse reaction in the operative rate was signiifcantly lower than that of the control group (P<0.05), the implementation of the study group patients after the surgery recovery quality is signiifcantly higher than the control group (P<0.05).Conclusion: for the elderly patients, during the operation by way of remifentanil combined with propofol for total intravenous anesthesia effect is good, has the advantages of high safety, less complications, signiifcant clinical effect, is worthy of promotion and application.
