摘要
古田街社区卫生服务中心,以"两个千方百计"为目标,切实做好家庭医生签约服务,提高门诊量,以期建立社区首诊的服务模式.对古田街社区卫生服务中心家庭医生签约服务工作的推进措施从三个方面进行介绍:一是高度重视、积极部署.组建适合中心运作的标准化团队;建立两个示范性团队逐步开展全人全程的健康管理服务;二是广泛宣传、深入动员.充分发挥中心跟居民的紧密性,积极跟街道居委会联系,建立起宣传网络;运用各种途径进行家庭医生签约工作的宣传;通过媒体扩大中心的影响力.三是明确原则、精细管理.明确团队责任;明确基本医疗与公共卫生不可分割;明确团队合作理念;明确科室分工纵向管理.从面临的主要问题进行分析:居民对签约接受度不够;签约后效果不佳;信息化的落后;缺乏相关政策的支持.并提出了落实签约服务的建议:加大政策宣传和解释力度;提高全科医生的综合素质,并建立健全的家庭医生服务团队;加强信息化的建设;借助医联体,形成初步的社区首诊制;夯实签约服务内涵等.
Abstract
With the goal of trying to practically implement signing service of family doctor and improve the capacity of primary medical service by all means, Gutian Street Community Health Service Center (the Center) aims to practically implement signing service of family doctor and increase the quantity of outpatient service so as to establish community-based first diagnosis service mode. This paper introduced the measures used to promote the signing service of family doctor in the Center from the following three aspects. Firstly, pay high attention to the signing service and actively conduct relevant deployment. Set up a standardized team suitable for the operation of the Center, and establish two demonstration teams to gradually carry out whole-person and whole-course health management service;Secondly, implement extensive publicity and mobilization. Give full play to the close relationship between the Center and residents and actively communicate with the neighborhood committees, so as to establish a publicity network. Make the signing service of family doctors public through various channels, and expand the Center's influence through the media. Thirdly, clarify related principles and implement fine management. Define team responsibilities and make it clear that basic medical care is inseparable from public health. Define the concept of teamwork and clarify the vertical management of responsibilities of various departments. Then the main problems were analyzed including inadequate acceptance of residents, small effects after signing, backward informatization and inadequate support of related policies. Finally, suggestions for the implementation of the signing service were proposed including strengthening policy publicity and explanation, improving the comprehensive quality of general practitioners and establishing improved family doctor service teams, improving the construction of informatization, forming a preliminary system of community-based first diagnosis mode by virtue of medical association, and consolidating the connotation of signing services.
