Different Acupuncture Methods for the Treatment of Postherpetic Neuralgia:A Network Meta-analysis

目的:采用网状Meta分析以评价不同针刺方法治疗带状疱疹后遗神经痛的有效性.方法:计算机检索PubMed、Web of science、国家知识基础设施数据库(CNKI)、中国学术期刊数据库(CSPD)、中文科技期刊数据库(CCD)、中国生物医学文献数据库(CBM)中针刺治疗带状疱疹后遗神经痛的随机对照试验,检索时限为建库至2021年8月15日.采用Cochrane偏倚风险评估工具对纳入文献进行筛选和质量评价,运用Rev Man5.4和Stata16.0进行数据分析.结果:纳入文献31篇,涉及13种干预措施,共计2330例患者,其中观察组1 120例,对照组1 210例.网状Meta分析结果显示,各干预措施疗效概率从高到低排序为:火针联合刺络拔罐(0.903)＞浮针联合火针(0.799)＞电针联合围刺(0.72)＞围刺联合刺络拔罐(0.619)＞浮针联合刺络拔罐(0.602)＞电针联合刺络拔罐(0.591)＞浮针(0.518)＞电针联合浮针(0.484)＞火针(0.477)＞刺络拔罐(0.467)＞围刺(0.206)＞电针(0.092)＞常规西药(0.023).结论:针刺治疗带状疱疹后遗神经痛具有一定优势,火针联合刺络拔罐疗效最佳.
Objective:To evaluate the efficacy of different acupuncture methods for the treatment of postherpetic neuralgia by net-work Meta-analysis.Methods:The randomized controlled trials(RCTs)regarding different acupuncture methods for the treatment of postherpetic neuralgia were searched in PubMed,Web of science,China National Knowledge Infrastructure(CNKI),Chinese Sci-ence Citation Database(CSPD),Chinese Citation Database(CCD)and China Biology Medicine disc(CBM)from the inception of the databases to August 15,2021.Quality of the literature was assessed using the risk of bias tool in the Cochrane Handbook,and RevMan 5.4 and Stata 16 were used for data analysis.Results:A total of 31 RCTs were included,involving 13 intervention meas-ures and 2 330 patients,1 120 in the observation group and 1 210 in the control group.The network Meta-analysis showed that the efficacy probability of different methods was ranked from high to the low as fire needle+collateral-bloodletting puncture and cup-ping(0.903)＞floating needling+fire needle(0.799)＞electroacupuncturecircumpuncture(0.72)＞circumpuncture+collateral-bloodletting puncture and cupping(0.619)＞floating needling+collateral-bloodletting puncture and cupping(0.602)＞electroacu-puncture+collateral-bloodletting puncture and cupping(0.591)＞floating needling(0.518)＞electroacupuncture+floating nee-dling(0.484)＞fire needle(0.477)＞collateral-bloodletting puncture and cupping(0.467)＞circumpuncture(0.206)＞electroa-cupuncture(0.092)＞conventional western medicine(0.023).Conclusion:Acupuncture is favorable in the treatment of posther-petic neuralgia,and fire needle combined with collateral-bloodletting puncture and cupping has the optimal clinical efficacy.
