基于文献与清代医案的半夏泻心汤证因机辨析

Analysis on the Causes and Mechanisms of Banxia Xiexin Decoction Based on Documents and Medical Cases in Qing Dynasty

经典名方半夏泻心汤出自《伤寒论》,古往医家对此方论述颇丰,辨证论治而取效于临床.每首方剂皆有其相应的证机,病机作为疏通中医古今异病同治的枢纽,贯穿于病脉证并治动态诊疗全过程.现通过梳理历代半夏泻心汤的理论文献,研究清代医家应用此方的医案,从理论到临床辨析半夏泻心汤证的病因与病机,并以Cytoscape、TBtools软件进行可视化,为研究经方古为今用提供参考思路.半夏泻心汤证理论文献与医案阐述的因机互有异同,清代医家临证应用多种辨证方法合而辨机,较为重视体质致病的影响,体现中医圆机活法的论治思维.
The classic prescription of Banxia Xiexin decoction comes from Treatise on Febrile Diseases.Ancient doctors have dis-cussed the prescription quite extensively.Its treatment based on syndrome differentiation is effective in clinical practice.Each pre-scription has its corresponding syndrome and mechanism.Pathogenesis can be used as the key of understanding"treating different diseases with same methods"in ancient and modern Chinese medicine,running through the whole process of the dynamic diagnosis and treatment of disease,pulse and syndrome.By reviewing the theoretical literature of Banxia Xiexin decoction in all dynasties,and studying the medical cases of its application by doctors in the Qing Dynasty,we analyzed the etiology and pathogenesis of Banxia Xiexin decoction syndrome from theory to clinic.Also,it was visualized with Cytoscape and TBtools software.As a result,the refer-ence ideas for the study of making the past classic prescription serving the present could be provided.There are similarities and differences for the causes and mechanisms descriptions between the theoretical literature and medical cases of Banxia Xiexin decoc-tion syndrome.In the Qing Dynasty,doctors applied a variety of syndrome differentiation methods to expand the indication of syn-drome and cause,paid more attention to the influence of constitution causing diseases,reflecting the TCM lively treatment thinking.
