中医手法治疗腰椎间盘突出症的研究现状与展望

摘要
腰椎间盘突出症(LDH)是骨伤科常见病、多发病,以腰痛和下肢放射痛为主要特征,属中医学"腰腿痛""痹证"范畴.中医手法是治疗本病的常用手段,具有缓解疼痛、改善局部功能的效应,因其疗效确切、操作简便和安全性高等优势在临床中被广泛使用.近年来,中医手法治疗LDH临床评价与机制研究方面取得了长足进步,但仍存在欠规范的问题,制约了中医手法的临床推广和应用.随着多学科交叉融合发展,医工结合研究模式为解决中医手法研究难题提供了有效途径.鉴于此,本文重点从临床和基础研究等方面对常用中医手法治疗LDH进行系统梳理,归结手法研究的不足,并针对存在的问题提出解决思路和方法,为后续手法规范化研究提供参考依据,助力中医药现代化研究进程.
Abstract
Lumbar disc herniation(LDH)is a common and frequently-occurring disease in orthopedics and traumatology.With low back pain and lower limb radiating pain as the main manifestations,LDH belongs to the category of"lumbocrural pain"and"im-pediment"in traditional Chinese medicine(TCM).TCM manual therapy is a commonly used method for treating LDH,with the main purpose of alleviating pain and improving local function,which is widely used in clinical practice due to its definite efficacy,simple operation,and high safety.In recent years,significant progress has been achieved in the clinical evaluation and mechanism research of TCM manual therapy for the treatment of LDH.However,the lack of standards limits the clinical promotion and applica-tion of TCM manual therapy.With the development of interdisciplinary integration,the research model combining medicine and en-gineering provides an effective way to solve the problems in the research on TCM manual therapy.This paper systematically sorted out the commonly used TCM manual therapies for LDH in clinical and basic studies.After summarizing the shortcomings,we pro-posed the corresponding solutions,providing a basis for subsequent research on the standardization of manual therapy and assisting the modern research of TCM.
