摘要
腰椎间盘突出症(LDH)是与人口老龄化密切相关的脊柱退行性疾病,运动疗法是LDH非手术治疗的重要组成部分.首先从不同运动类型治疗LDH的临床研究入手,涵盖了各种静态运动、有氧运动和中医传统运动,分析其在不同疾病阶段的适用性及应用效果.其次,基于筋-骨-肌肉理论、微环境生理生化和中枢脑功能探讨不同运动疗法在LDH治疗中的作用.综合来看,运动疗法对于LDH的疾病管理具有广阔的发展前景,因此倡导在未来的研究中结合高质量循证医学证据和多维度现代科学技术,制定更为精准的治疗方案,最大程度地实现临床转化价值,同时,推进运动疗法的标准化方案,进一步证实运动疗法的疗效、安全性及作用机制,是今后的努力方向.
Abstract
Lumbar disc herniation(LDH)is a degenerative disease closely related to aging,for which exercise is an important non-surgical therapy.We first reviewed the clinical studies on the treatment of LDH with different exercise therapies,which covered stat-ic exercises,aerobic exercises,and traditional Chinese exercises,and then analyzed the applicability and application effects of these therapies in different disease stages.Secondly,we probed into the mechanisms of different exercise therapies based on the tendon-bone-muscle theory,microenvironmental physiological and biochemical mechanism,and central nervous system(brain)function.From a comprehensive perspective,exercise therapy has a broad prospect for the management of LDH.Therefore,it is advocated to combine high-quality medical evidence with multidimensional modern technology to develop accurate treatment plans and maximize the clinical translational value.Meanwhile,promoting the standardized program of exercise therapy and revealing the efficacy,safe-ty,and mechanism of exercise therapy is the direction of future efforts.
