摘要

糖尿病认知功能障碍属于"消渴呆病"范畴,其病机本虚标实,以脏腑亏虚为本,痰瘀糖毒阻络为标.玄府、络脉是人体普遍存在的一种微观结构,具有维系周身气血津液运行的功能,玄府闭塞、络脉阻滞是多种疾病发生的病理基础.基于"玄府-络脉"的新视角,分析其与糖尿病认知功能障碍发病的关联性.玄府闭塞,脑络不通,神机失用是糖尿病认知功能障碍的病机,治宜以通为用,临证组方借虫类风药开玄通络,共增药效,为糖尿病认知功能障碍的临床治疗提供思路.

Abstract

Diabetic cognitive impairment belongs to the category of"mental deterioration due to consumptive thirst"in traditional Chinese medicine(TCM).Its pathogenesis is deficiency in root causes and excess in manifestations,with organ deficiency as the root and phlegm,stasis,and sugar toxin obstructing collaterals as the manifestation.Sweat pore and collateral vessels are microstruc-tures universally present in the human body,with the function of maintaining the circulation of qi,blood,and body fluids throughout the body.Occlusion of sweat pore and blockage of collateral vessels serve as the pathological basis for the occurrence of various dis-eases.From the new perspective of"sweat pore-collateral vessels",this study analyzed its correlation with the onset of diabetic cog-nitive impairment.The vital activity dysfunction resulting from occlusion of sweat pore and the blockage of collateral vessels in the brain is the pathogenesis of diabetic cognitive impairment.Treatment should focus on dredging.Clinically,formulas based on wind-related drugs derived from insects are adopted to open sweat pore and dredge collaterals,which can enhance the efficacy of treat-ment,providing insights into the clinical treatment of diabetic cognitive impairment.
