面向"肝脾同调、疏运建中"诠释胃癌前状态诊疗教学的创新优化

胃黏膜萎缩和肠化生已于2020年被归类为胃癌前状态,成为一种新的疾病分类。应对疾病谱-现代生活方式的改变,倡导"肝脾同调、疏运建中"的学术观点,创新性丰富"肝浊"的理论,深化了肝郁浊毒论。以病证思维为主线,指导临床实践,确立治疗宜以"握枢持中不忘疏肝,防微杜渐不忘补肾,开门祛邪不忘化浊"为治疗原则,并针对此则,创制基于柴胡疏肝散与平胃散的"新肝脾九味药方";关注炎-癌转化,实践"长期看病不等于长期吃药,慢病管理不等于慢病治疗"的新型管理模型。
Innovative Optimization of Diagnosis and Treatment Teaching for Gastric Precancerous States Based on"Liver and Spleen Harmonization,Dredging and Construction of Middle Jiao"
Gastric mucosa atrophy and intestinal metaplasia have been classified as gastric precancerous states in 2020,contribu-ting to a new disease classification.In response to the change in disease spectrum and modern lifestyle,the academic viewpoint of"liver and spleen harmonization,dredging and construction of middle jiao"was advocated,and the"liver turbidity"was innovative-ly enriched to deepen the theory of"liver stagnation leads to turbidity and toxin".By taking the disease and syndrome thinking as the main line,clinical practice was guided,and the treatment principle of"following the regularity with dredging the liver,nipping in the bud with tonifying the kidney,and eliminating external evils with relieving the turbidity"was established.On this basis,the Xinganpi Jiuwei Prescription based on Chaihu Shugan Powder and Pingwei Powder was developed.It is necessary to pay attention to the transformation of inflammation to cancer and implement the new management model that"long-term medical treatment may not take long-term medicine,and chronic disease management cannot be considered as chronic disease treatment".
