基于脑肠互动探析针刺治疗功能性便秘的作用机制

功能性便秘是多因素、多机制参与的难治性胃肠疾病,脑肠轴失衡被认为是其核心病机。脑-肠轴作为脑与胃肠道的双向调节通道,参与其他发病机制的作用过程,贯穿疾病始终。通过梳理脑-肠轴与功能性便秘的关系,从针刺调节神经系统、脑-肠肽、肠道菌群、下丘脑-垂体-肾上腺轴以及中医治疗的理论依据出发,多角度探析针刺通过脑-肠轴治疗功能性便秘的可能作用机制,以期优化临床治疗方案,提高疗效。
Mechanism of Acupuncture in Treatment of Functional Constipation Based on Brain-gut Axis Theory
Functional constipation is a refractory gastrointestinal disorder influenced by multiple factors and mechanisms,with an imbalance in the brain-gut axis considered a core pathological mechanism.The brain-gut axis serves as a bidirectional regulatory pathway between the brain and gastrointestinal tract,playing a role throughout the disease process.By exploring the relationship be-tween the brain-gut axis and functional constipation,and examining how acupuncture regulated the nervous system,brain-gut pep-tides,gut microbiota,the hypothalamic-pituitary-adrenal(HPA)axis,as well as traditional Chinese medicine(TCM)theories,this review elucidated the underlying mechanisms through which acupuncture treated functional constipation via the brain-gut axis,ai-ming to optimize clinical treatment strategies and enhance therapeutic efficacy.
