血管性痴呆(vascular dementia,VD)是一种常见的神经退行性疾病,其主要表现为痴呆、智力普遍下降.近年来全球血管性痴呆患者的发病率和死亡率逐渐上升,且临床上尚无有效治疗方案.越来越多研究表明,核因子E2相关因子2(nuclear factor E2-related factor 2,NRF2)在血管性痴呆中发挥重要作用,参与调节人体抗氧化应激防御系统,其激活可增强血管性痴呆的抗氧化能力,在VD病理多个方面起到关键作用.NRF2参与调控VD损伤的氧化应激反应,降低神经元铁死亡,抑制小胶质细胞的神经炎症反应,维持线粒体的正常功能,保持血脑屏障的完整性.因此,本文从氧化应激、神经炎症、铁死亡、线粒体损伤、血脑屏障等几个方面入手,探讨NRF2在血管性痴呆中作用机制及研究进展,以期为VD的治疗提供新思路.
Mechanism of transcription factor NRF2 in vascular dementia
Vascular dementia(VD)is a common neurodegenerative disease that is characterized by dementia and a general decline in mental ability.In recent years,the morbidity and mortality of VD have been increasing worldwide,and there are no effective clinical treatment options.A growing number of studies have shown that nuclear factor E2-related factor 2(NRF2)plays an important role in vascular dementia and is involved in regulating the body's antioxidant stress defense system,and its activation enhances the antioxidant capacity of vascular dementia and plays a key role in several aspects of VD pathology.NRF2 is involved in regulating the oxidative stress response to VD injury,reducing neuronal iron death,inhibiting the neuroinflammatory response in microglia,maintaining normal mitochondrial function,and preserving the integrity of the blood-brain barrier.Therefore,this paper reviews the mechanism and research progress of NRF2 in vascular dementia from several aspects,including oxidative stress,neuroinflammation,iron death,mitochondrial damage,and blood-brain barrier,in order to provide new ideas for the treatment of VD.
