摘要

目的 探讨分析颅脑损伤患者行CT与MRI诊断的临床价值.方法 选择南京市溧水区人民医院2021年1月至2023年11月收治的97例疑似颅脑损伤患者,入院均接受CT、MRI检查.以综合诊断结果为金标准,对比两种检查方式的诊断效能、颅脑损伤类型及不同血肿厚度的诊断符合率.结果 97例疑似颅脑损伤,最终确诊82例.MRI诊断颅脑损伤的特异度、灵敏度和准确度分别为86.67%、95.12%和93.81%,明显高于CT的53.33%、85.36%及80.41%(x2=0.046、0.035、0.005,P<0.05);MRI检测与CT检测在各型颅脑损伤诊断中无明显差异(P>0.05);在诊断<1.5 mm血肿中的符合率明显高于CT(P<0.05).结论 CT与MRI均可诊断颅脑损伤,但MRI诊断的符合率更高,可提高微小血肿诊断率,值得临床应用.

Abstract

Objective:To analyze the clinical value of CT and MRI in patients with craniocerebral injury.Methods:A total of 97 patients with suspected head injury admitted to Nanjing Lishui District People's Hospital from January 2021 November 2023 were selected and all of them underwent CT and MRI examinations.Taking the comprehen-sive diagnosis results as the gold standard,the diagnostic efficacy of the two examination methods,the type of head injury disease and the diagnostic coincidence rate of different hematoma thicknesses were compared.Results:There were 97 cases of suspected head injury,and 82 cases were finally diagnosed.The specificity,sensitivity and accuracy of MRI in the diagnosis of head injury were 86.67%,95.12%and 93.81%,respectively,which were significantly higher than those of CT 53.33%、85.36%and 80.41%(x2=0.046、0.035、0.005,P<0.05),the coincidence rate of 1.5 mm hematoma was significantly higher than that of CT(P<0.05).Conclusion:CT and MRI can diagnose head injury,but MRI has a higher coincidence rate and can improve the diagnosis rate of microhematoma,which is worthy of clinical application.
