超声骨密度检测在膝关节骨性关节炎患者骨质疏松症筛查中的应用分析

摘要
目的 分析超声骨密度检测在膝关节骨性关节炎患者骨质疏松症筛查中的应用效果.方法 依据Kellgren-Lawrence标准,将160例膝关节骨关节炎患者划分为Ⅰ、Ⅱ、Ⅲ、Ⅳ四级,记录患者身高、体重、年龄及超声骨密度相关数据,分析超声骨密度检测的T值,评价超声骨密度检测在骨质疏松症筛查中的准确性,探讨超声骨密度筛查值大小与膝关节骨关节炎的相关性.结果 随着膝关节骨性关节炎分级的升高,患者超声骨密度的T值趋向负值,提示其病情恶化,这一变化情况在统计学上差异显著(X2=117.722,P<0.001).超声骨密度与与CT骨密度之间存在显著正相关关系(r=0.751,P<0.05).将超声骨密度T值<-2.5作为骨质疏松症的判断标准时,其诊断的曲线下面积(AUC)达到0.774,95%,置信区间在0.676~0.791之间,特异度为96.7%,灵敏度为85.7%,约登指数为82.4%.随着Kellgren-Lawrence分级的提高,即膝关节骨关节炎病情的恶化,患者的T值向负值方向增大.结论 超声骨密度检测具有特异度和敏感度高的特点,适用于膝关节骨性关节炎患者骨质疏松症的筛查.患者骨关节炎程度越严重,发生骨质疏松症的可能性越大.
Abstract
Objective:To analyze the application effect of ultrasonic bone mineral density detection in screening osteo-porosis in patients with knee osteoarthritis.Methods:According to Kellgren-Lawrence standard,160 patients with knee osteoarthritis were divided into four grades Ⅰ、Ⅱ、Ⅲ、Ⅳ.The data of patients'height、weight、age and ultra-sonic bone density were recorded,the T value of ultrasonic bone density detection was analyzed,the accuracy of ul-trasonic bone density detection in osteoporosis screening was evaluated,and the correlation between ultrasonic bone density screening value and knee osteoarthritis was discussed.Results:With the increase of knee osteoarthritis grade,the T value of ultrasonic bone mineral density of patients tended to be negative,suggesting that their condi-tion worsened,and the change was statistically significant(x2=117.722,P<0.001).There is a significant positive correlation between ultrasound BMD and CT BMD(r=0.751,P<0.05).When the T value of ultrasonic bone min-eral density<-2.5 is taken as the criterion forjudging osteoporosis,the area under the curve(AUC)of the diagno-sis is 0.774,95%,the confidence interval is 0.676~0.791,the specificity is 96.7%,the sensitivity is 85.7%,and the Jordan index is 82.4%.With the improvement of Kellgren-Lawrence classification,that is,the deterioration of knee osteoarthritis,the patient's t value increases in the negative direction.Conclusion:Ultrasonic bone mineral den-sity detection has the characteristics of high specificity and sensitivity,and is suitable for screening osteoporosis in patients with knee osteoarthritis.The more severe the degree of osteoarthritis,the greater the possibility of osteo-porosis.
