摘要

目的 研究血气乳酸、血管紧张素转换酶(ACE)及APACHEⅡ评分预测急诊创伤性休克患者死亡风险的价值.方法 收集2018年3月至2023年5月桐城市人民医院重症医学科收治的70例急诊创伤性休克患者,分为存活组(38例)与死亡组(32例).比较两组乳酸、ACE、ACE2、APACHEⅡ评分、ISS及SI,分析上述指标在预测急诊创伤性休克患者死亡风险中的价值.结果 存活组乳酸、ACE、ACE2、APACHEⅡ评分及SI均低于死亡组,P<0.05;Logistic回归分析显示,NLR、乳酸、ACE、ACE2、APAC HEⅡ评分是急诊创伤性休克患者预后的影响因素,P<0.05);联合预测死亡风险的ROC面积为0.956,敏感度0.925,特异度0.756.结论 乳酸、ACE及APACHEⅡ评分高的急诊创伤性休克患者死亡风险较高,应用三项指标联合检测具有较高的临床价值.

Abstract

Objective:To study the value of blood gas lactic acid,angiotensin converting enzyme(ACE)and A-PACHEⅡ score in predicting the death risk of emergency patients with traumatic shock.Methods:From March 2018 to May 2023,70 emergency patients with traumatic shock were collected and divided into survival group(38 ca-ses)and death group(32 cases).Lactic acid、ACE、ACE 2、APACHEⅡ score、ISS and SI were compared between the two groups,and the value of the above indexes in predicting the death risk of emergency patients with traumatic shock was analyzed.Results:Lactic acid、ACE、ACE2、APACHEⅡ score and SI in survival group were lower than those in death group,P<0.05.Logistic regression analysis showed that the scores of NLR、lactic acid、ACE、ACE2 and APACHEⅡ were the prognostic factors of emergency patients with traumatic shock,P<0.05.The ROC area of joint prediction of death risk is 0.956,the sensitivity is 0.925,and the specificity is 0.756.Conclusion:Emergency patients with traumatic shock with high scores of lactic acid、ACE and APACHEⅡ are at higher risk of death,and the combined detection of these three indexes has high clinical value.
