目的 探讨分析有创-无创序贯性机械通气治疗老年肺内源性急性呼吸窘迫综合征(ARDS)患者的临床效果.方法 选取 2022 年 1 月至 2023 年 9 月东莞市大朗医院重症医学科收治的肺内源性 ARDS老年患者 60 例,根据电脑随机法分为对照组(30 例)与实验组(30 例).两组均给予气管插管机械通气,对照组采用气管插管或气管切开治疗,实验组采用有创-无创序贯性机械通气治疗.对比两组血气分析指标[包括动脉血氧分压(PaO2)、动脉血二氧化碳分压(PaCO2)]、有创机械通气时间、总通气时间、ICU 治疗时间、呼吸机相关性肺炎发生率及病死率.结果 经过治疗,相比对照组,试验组PaO2 水平更高,PaCO2 水平更低,P<0.05.相比对照组,试验组进行有创机械通气时间、总通气时间与ICU治疗时间均更短,P<0.05.相比对照组,试验组呼吸机相关性肺炎发生率与病死率对比均更低,P<0.05.结论 采用有创-无创序贯性机械通气治疗老年肺内源性 ARDS 患者效果显著,可改善患者血气分析指标,减轻症状,缩短通气时间与ICU治疗时间,并可降低患者呼吸机相关性肺炎发生率与病死率.
Effect of Invasive-noninvasive Sequential Mechanical Ventilation on Elderly Patients with Pulmonary Endogenous Acute Respiratory Distress Syndrome
Objective:To explore and analyze the clinical effect of invasive-noninvasive sequential mechanical venti-lation in the treatment of elderly patients with pulmonary endogenous acute respiratory distress syndrome(ARDS).Methods:From January 2022 to September 2023,60 elderly patients with pulmonary endogenous ARDS admitted to the Department of Critical Care Medicine of Dongguan Da Lang Hospital were randomly divided into control group(30 cases)and experimental group(30 cases).Both groups were given tracheal intubation and mechanical ventila-tion,while the control group was treated with tracheal intubation or tracheotomy,while the experimental group was treated with invasive-noninvasive sequential mechanical ventilation.The blood gas analysis indexes[including arte-rial partial pressure of oxygen(PaO2)and arterial partial pressure of carbon dioxide(PaCO2)]、invasive mechanical ventilation time、total ventilation time、ICU treatment time、incidence and mortality of ventilator-associated pneu-monia were compared between the two groups.Results:After treatment,compared with the control group,the PaO2 level in the experimental group was higher and the PaCO2 level was lower,P<0.05.Compared with the control group,the time of invasive mechanical ventilation、total ventilation and ICU treatment in the experimental group were shorter,P<0.05.Compared with the control group,the incidence and mortality of ventilator-associated pneumonia in the experimental group were lower,P<0.05.Conclusion:Invasive-noninvasive sequential mechanical ventilation is effective in the treatment of elderly patients with pulmonary endogenous ARDS,which can improve blood gas analysis indexes,relieve symptoms,shorten ventilation time and ICU treatment time,and reduce the incidence and mortality of ventilator-associated pneumonia.
