补阳还五汤对脑卒中患者脑侧支循环建立及免疫功能的影响

摘要
目的 探究补阳还五汤对脑卒中患者脑侧支循环建立及免疫功能的影响.方法 研究时段设定为 2022 年 1-12 月,研究对象为上述时段中,湖南中医药大学第二附属医院收诊脑卒中确诊病例 68 个,参照随机信封原则开展对照、观察组别划分,均设定 34 例.对症西药为对照组研究方案,对症西药+补阳还五汤为观察组研究方案,对比两组脑侧支循环建立情况、免疫功能、神经功能、认知功能、吞咽功能、昏迷程度及中医证候积分.结果 治疗 7d、1个月后,观察组脑侧支循环改善率均高于对照组,P<0.05.治疗第 15d,观察组CD4、CD8 及 CD4/CD8 均明显高于对照组高,P<0.05.治疗后,观察组言语謇涩、口舌歪斜及半身不遂积分均低于对照组,P<0.05;观察组NIHSS、SSA评分均低于对照组,MMSE及GCS评分均较对照组高,P<0.05.结论 对脑卒中患者在常规治疗基础上增加补阳还五汤可有效改善患者脑侧支循环状况,提高其免疫功能、神经功能、认知功能、吞咽功能,减轻昏迷程度,同时可明显改善患者中医证候.
Abstract
Objective:To explore the effect of Buyanghuanwu decoction on the establishment of cerebral collateral circulation and immune function in stroke patients.Methods:The research period is set from January to December 2022,and the research subjects are 68 confirmed cases of stroke admitted to the Second Affiliated Hospital of Hu-nan University of Traditional Chinese Medicine during the above period.The control and observation groups are di-vided according to the principle of random envelope,with 34 cases in each group.Symptomatic Western Medicine was used as the control group research plan,and Symptomatic Western Medicine+Buyang Huanwu Decoction was used as the observation group research plan.The establishment of cerebral collateral circulation,immune function,neurological function,cognitive function,swallowing function,coma degree,and traditional Chinese medicine syn-drome score were compared between the two groups.Results:After 7 days and 1 month of treatment,the improve-ment rate of cerebral collateral circulation in the observation group was higher than that in the control group,P<0.05.On 15th day of treatment,CD4、CD8 and CD4/CD8 in the observation group were significantly higher than those in the control group,P<0.05.After treatment,the scores of unsmooth speech、crooked mouth and tongue and hemiplegia in the observation group were lower than those in the control group,P<0.05.The scores of NIHSS and SSA in the observation group were lower than those in the control group,while the scores of MMSE and GCS were higher than those in the control group,P<0.05.Conclusion:Adding Buyanghuanwu decoction to stroke pa-tients on the basis of routine treatment can effectively improve their cerebral collateral circulation,improve their im-mune function,neurological function,cognitive function and swallowing function,reduce the degree of coma,and obviously improve their TCM syndromes.
