摘要
目的 探讨分析青少年颌骨囊肿治疗中采用开窗减压术与囊肿刮治术的临床效果.方法 纳入南充市中心医院收治的 70 例青少年颌骨囊肿患者(2022 年 1 月至 2023 年 10 月),以随机数字表法分为对照组与观察组,各 35例.对照组行囊肿刮治术,观察组行开窗减压术.对比两组手术指标、预后指标、疼痛介质水平、并发症及复发率.结果 较对照组,观察组手术时间更短,术中出血量更少,术后 24hVAS 评分更低,P<0.05.较对照组,骨质增生厚度、骨密度、囊腔体积缩小程度,均观察组更高,囊肿愈合用时,观察组更短,P<0.05.较对照组,疼痛介质 SP、NPY、NGF水平,均观察组更低,P<0.05.较对照组,术后并发症,观察组更少,P<0.05.术后 6 个月复发率,2 组无差异,P>0.05.结论 开窗减压术治疗青少年颌骨囊肿可减少出血量,缩短手术时间及术后愈合时间,且预后指标良好,疼痛轻,并发症少,复发率低.
Abstract
Objective:To explore and analyze the clinical effect of fenestration decompression and cyst curettage in the treatment of juvenile jaw cyst.Methods:70 adolescent patients with jaw cyst admitted to Nanchong Central Hospital(January 2022 to October 2023)were enrolled and divided into control group and observation group by random number table method,with 35 cases in each group.The control group was treated with curettage of cyst,and the observation group was treated with fenestration and decompression.The operation index、prognosis index、pain medium level、complications and recurrence rate were compared between the two groups.Results:Compared with control group,observation group had shorter operation time,less blood loss and lower VAS score 24 h after operation(P<0.05).Compared with the control group,the thickness of bone hyperplasia,bone density and vol-ume reduction of cyst were higher in observation group,and the healing time of cyst was shorter in observation group(P<0.05).Compared with control group,the levels of pain mediators SP,NPY and NGF were lower in observation group(P<0.05).Compared with the control group,the postoperative complications were less in the observation group(P<0.05).The recurrence rate of 6 months after operation was not different between the two groups(P>0.05).Conclusion:Decompression by fenestration can reduce the amount of bleeding,shorten the oper-ation time and postoperative healing time,with good prognosis,light pain,few complications and low recurrence rate.
