摘要

目的 探讨分析动静脉二氧化碳分压差在脓毒性休克液体复苏中的应用效果.方法 选择性分析 2022 年 8 月至2024 年 4 月于佛山市顺德区第五人民医院行脓毒性休克液体复苏治疗的 96 例患者的临床资料,按动静脉二氧化碳分压差高低分组,标准值为 6 mmHg,低压组与高压组各 48 例.对两组预后效果进行对比.结果 复苏前,两组 A-PACHEⅡ评分、SOFA评分、血流动力学指标对比,P>0.05;复苏后,两组APACHEⅡ评分、SOFA评分、血流动力学指标均优于复苏前,两组MAP、CVP、ScrO2 HR对比,P>0.05;低压组APACHEⅡ评分、SOFA评分、HR、Pcv-aCO2 均较高压组更低,P<0.05.低压组 24h液体摄入量少于高压组,机械通气时间与住ICU时间均短于高压组,P<0.05.低压组病死率为 10.42%,低于高压组的 37.50%,P<0.05.结论 动静脉二氧化碳分压差在脓毒性休克液体复苏中的应用价值较高,值得临床广泛运用.

Abstract

Objective:To explore and analyze the application effect of arterial and venous carbon dioxide partial pressure difference in fluid resuscitation of septic shock.Methods:The clinical data of 96 patients who were treated with fluid resusci-tation for septic shock in the Fifth People's Hospital of Shunde District,Foshan City from August 2022 to April 2024 were selectively analyzed.They were divided into two groups according to the venous carbon dioxide partial pressure difference,with the standard value of 6 mmHg,48 cases in the low pressure group and 48 cases in the high pressure group.The prog-nosis effects of the two groups were compared.Results:Before resuscitation,APACHEⅡ score、SOFA score and hemody-namic indexes were compared between the two groups,P>0.05;After resuscitation,the APACHEⅡ ⅱ score、SOFA score and hemodynamic indexes of the two groups were better than those before resuscitation,and the comparison of MAP、CVP and ScrO2 HR between the two groups was,P>0.05.APACHEⅡ score、SOFA score、HR and Pcv-aCO2 in low pressure group were lower than those in high pressure group,P<0.05.The 24-hour fluid intake of low-pressure group was less than that of high-pressure group,and the time of mechanical ventilation and ICU stay were shorter than that of high-pressure group,P<0.05.The mortality rate of low pressure group was 10.42%,which was lower than that of high pres-sure group 37.50%,P<0.05.Conclusion:The application value of arterial and venous carbon dioxide partial pressure difference in fluid resuscitation of septic shock is high,which is worthy of wide clinical application.
