To Investigate the Implementation of a'doctor-nurse-patient'Collaborative Nursing Model based on SMART for the Home Care of Stroke Patients with Cognitive Impairment
Objective:To explore the application effect of smart-based"doctor-patient"collaborative nursing mod-el in home management of stroke patients with cognitive impairment.Methods:100 patients with cognitive impair-ment after stroke admitted to the Department of Neurology of Rugao People's Hospital from January 2020 to De-cember 2023 were randomly divided into control group and observation group,with 50 cases in each group.The con-trol group was given routine nursing and discharge guidance before discharge,and the observation group was given the"doctor-patient"collaborative nursing mode based on SMART.The cognitive function,quality of life and self-care ability of daily living were compared between the two groups before and after intervention for 2 months.Re-sults:After the intervention,the MMSE and MBI scores of the observation group were higher than those of the con-trol group,and the SIS score was lower than that of the control group,P<0.05.Conclusion:Implementing the"doctor-patient"collaborative nursing model based on SMART for patients with cognitive impairment after stroke can effectively restore their cognitive function and improve their self-care ability and quality of life.
