Predictive Value of Serum pro-ADM,Galectin-3 Combined with TLR-4 for Histological Chorioamnionitis in Pregnant Women with Preterm Premature Rupture of the Membrane
Objective:To analyze the predictive value of adrenal-adrenomedullin precursor(pro-ADM),galectin-3(Galectin-3)and Toll-like receptor 4(TLR-4)for histological chorioamnionitis(HCA)in pregnant women with preterm premature rupture of the membrane(PPROM).Methods:The study included 160 pregnant women with PPROM who were admitted to the obstetrics department of The Affiliated Hospital of Xuzhou Medical University and Friendship Hospital of Xinjiang Ili Kazakh Autonomous Prefecture from June 1,2021 to June 1,2023 as the research subjects.According to the pathological results of postpartum placenta in PPROM pregnant women,they were divided into HCA group(n=71)and non-HCA group(n=89).The levels of serum pro-ADM,Galectin-3 and TLR-4 in pregnant women were detected by enzyme-linked immunosorbent assay,the related factors of HCA between groups were analyzed by univariate and multivariate Logistic analysis,and the predictive value of the model were analyzed by receiver operating characteristic curve(ROC)curve.Results:The results of univariate analysis showed that,the proportion of multipara and cesarean section in HCA group was higher than that in non-HCA group,and the levels of serum PCT,CRP,pro-ADM,Galectin-3 and TLR-4 were higher than those in non-HCA group(P＜0.05).Logistic regression analysis showed that,PCT,CRP,pro-ADM,Galectin-3 and TLR-4 were indepen-dent risk factors for HCA in pregnant women with PPROM(P＜0.05).The ROC analysis model of the diagnostic efficiency of the influ-encing factors of HCA in PPROM pregnant women was established.The area under the curve(AUC)of pro-ADM,Galectin-3 and TLR-4 in independent and combined prediction was 0.720,0.743,0.790 and 0.874,the diagnostic efficiency of combined application were higher respectively.Conclusion:The elevated levels of serum pro-ADM,Galectin-3 and TLR-4 suggest an increased risk of HCA in pregnant women with PPROM,and the combination of the three has a higher predictive value for HCA disease in pregnant women with PPROM.
