Occurrence and influencing factors of postoperative hypoglycemia in patients undergoing elective endoscopic high-frequency electrocoagulation resection of colorectal polyps
Objective To analyze the occurrence and influencing factors of postoperative hypoglycemia in patients undergoing elective endoscopic high-frequency electrocoagulation resection of colorectal polyps.Methods From May 2020 to May 2022,a total of 200 patients performed elective endoscopic high-frequency electrocoagulation resection of colorectal polyps were enrolled,which included 104 males and 96 females,aged 34-77 years old with mean age of 50.6 years old;body mass in-dex(BMI)was 20.1-23.3 kg/m2 with mean BMI of 21.3 kg/m2.There were 92 cases of single polyps and 108 cases of mul-tiple polyps.Combined with type 2 diabetes in 45 cases,poor sleep quality in 59 and 66 of anxiety and depression.The in-cidence of hypoglycemia at 1-day after operation was counted.The patients with hypoglycemia were enrolled in hypo-glycemia group,and without hypoglycemia were enrolled in non-hypoglycemia group.The clinical data of patients were col-lected,and influencing factors of postoperative hypoglycemia were analyzed by Logistic regression.Results The hypo-glycemia occurred in 21 of 200 patients,the total incidence was 10.50%.The univariate analysis showed that age ≥ 60 years old,combined with type 2 diabetes,preoperative fasting time ≥ 10-hour,without fluid replacement,poor sleep quality,anxiety and de-pression,and postoperative unreasonable fluid replacement in hypoglycemia group were higher than those in non-hypoglycemia group(P<0.05).The multivariate Logistic regression analysis showed that age ≥ 60 years old,combined with type 2 diabetes,fasting time ≥ 10-hour,without rehydration,poor sleep quality,anxiety and depression,and postoperative unreasonable rehydration were all independent risk factors for hypoglycemia after elective endoscopic high-frequency electrocoagulation colorectal polyps(P<0.05).Conclusion It is demonstrated that the incidence rate of postoperative hypoglycemia is high in patients performed endoscopic high-frequency electrocoagulation resection of colorectal polyps,which is closely related to the age,history of hyperglycemia,pre-operative fasting time,preoperative and postoperative fluid replacement,preoperative sleep quality and anxiety and depression,and preventive measures could be designed to reduce the risk of postoperative hypoglycemia.
